摘　　要：目的探讨应用可溶性血管内皮生长因子受体-1(sFlt-1)联合子宫动脉搏动指数(PI)､阻力指数(RI)建立子痫前期预测模型,以便更好地开展对子痫前期的早期临床评估。方法选取2020年10月至2021年3月于华北石油管理局总医院常规产检的300例孕妇作为研究对象。其中20例发展为重度子痫前期,纳入重度组;45例发展为轻度子痫前期,纳入轻度组;235例未发生子痫前期,纳入对照组。比较三组孕妇的血清sFlt-1水平及子宫动脉PI与RI,并采用二项Logistic回归分析,建立联合预测因子模型。结果三组的血清sFlt-1水平､子宫动脉PI及RI比较,差异具有统计学意义(P<0.05),且轻度组高于对照组,重度组高于轻度组;联合预测拟合方程为:logit(P)=-70.218+0.007×sFlt-1+21.955×PI+82.298×RI;与妊娠结果比较,预测正确率为98.0%。结论联合预测拟合方程能够有效预测子痫前期的发生发展,对于更好地开展子痫前期的早期临床评估具有重要意义。Objective To establish a combined prediction model of preeclampsia using soluble fms-like tyrosine kinase-1(sFlt-1),uterine artery pulsatility index(PI),resistance index(RI)in order to better carry out the early clinical evaluation of preeclampsia.Methods 300 pregnant women underwent routine obstetric examinations in General Hospital of North China Petroleum Administration from October 2020 to March 2021 were selected as the research subjects.20 of them who developed severe preeclampsia were included in the severe group.45 cases who developed mild preeclampsia were included in mild group.235 cases without preeclampsia were included in the control group.Serum sFlt-1 levels and uterine artery PI and RI were compared among the three groups,and binomial Logistic regression analysis was used to establish a combined predictor model.Results The levels of serum sFlt-1,PI and RI of uterine artery in the control group,mild group and severe group increased in turn,and the differences were statistically significant(P<0.05).The combined prediction fitting equation:logit(P)=-70.218+0.007×sFlt-1+21.955×PI+82.298×RI.The total percentage of correct prediction was 98.0%.Conclusions The combined prediction fitting equation can effectively predict the development of preeclampsia,which is of great significance for better early clinical evaluation of preeclampsia.
