大连地区梅毒血清固定影响因素的临床流行病学研究

摘　　要：目的探讨大连地区梅毒患者血清固定的影响因素。方法选取2014年1月至2018年12月在大连市皮肤病医院性病科首诊或由大连地区其他医疗机构转诊的1507例梅毒患者作为研究对象,对性别､年龄､婚姻状况､梅毒分期､伴发性病､T细胞亚群､治疗用药､初始滴度､梅毒螺旋体IgM抗体检测(TP-IgM)､感染途径等因素进行Logistic回归分析,探讨梅毒血清固定的影响因素。结果437例梅毒患者发生血清固定,单因素Logistic回归分析显示,梅毒血清固定与梅毒分期､伴发性病､T细胞亚群､治疗用药有相关性(P<0.05);多因素Logistic回归分析显示,梅毒分期､初始滴度､TP-IgM､伴发性病､T细胞亚群､治疗用药为血清固定的影响因素(P<0.05)。结论梅毒分期､初始滴度､TP-IgM､伴发性病､T细胞亚群､治疗用药是影响大连地区梅毒患者血清固定的危险因素,需及早诊断,规范治疗,加强宣传力度,倡导健康生活,以防范梅毒血清固定的发生。Objective To explore the influencing factors of syphilis serofixation in Dalian area.Methods 1,507 syphilis patients who were first diagnosed in the Department of Venereology of Dalian Dermatosis Hospital or referred by other medical institutions in Dalian area from January 2014 to December 2018 were selected as the research subjects.Logistic regression analysis was carried out to investigate the influencing factors of serofixation of syphilis,including sex,age,marital status,syphilis stage,concomitant venereal disease,T cell subsets,treatment medication,initial titer,treponema pallidum IgM antibody detection(TP-IgM)and infection route.Results 437 patients with syphilis had serofixation.Univariate Logistic regression analysis showed that there was a correlation between serofixation and syphilis stage,associated venereal disease,T cell subsets and treatment drugs(P<0.05).Multivariate Logistic regression analysis showed that syphilis stage,initial titer,TP-IgM,concomitant venereal disease,T cell subsets and therapeutic drugs were the influencing factors of serofixation(P<0.05).Conclusions Syphilis stage,initial titer,TP-IgM,concomitant venereal disease,T cell subsets and therapeutic drugs are the risk factors affecting serofixation of syphilis patients in Dalian.It is necessary to strengthen publicity,advocate healthy life,early diagnosis and standardized treatment to prevent the occurrence of serofixation of syphilis patients.
