参芪健胃颗粒对慢性萎缩性胃炎治疗作用的Meta分析

摘　　要：目的系统评价参芪健胃颗粒治疗慢性萎缩性胃炎(CAG)的有效性和安全性。方法计算机检索中英文各数据库,检索时间为各数据库建库至2020年11月,纳入参芪健胃颗粒治疗CAG的随机对照研究(RCT),对纳入文献进行方法学质量评价,采用RevMan 5.4.1软件进行Meta分析。结果共纳入10个RCT,包括887例CAG患者。Meta分析结果显示:参芪健胃颗粒治疗CAG与对照组相比,可提高治疗总有效率[OR=4.04,95%CI(2.67~6.11),P<0.00001]以及Hp治疗有效率(OR=2.39,95%CI:1.47~3.89,P<0.001),改善中医证候[胃脘疼痛(MD=-0.67,95%CI:-0.85~-0.49,P<0.00001)、胃脘痞胀(MD=-0.59,95%CI:-0.79~-0.39,P<0.00001)、倦怠乏力(MD=-0.67,95%CI:-0.77~-0.56,P<0.00001)、食少纳呆(MD=-0.76,95%CI:-0.90~-0.62,P<0.00001)],提高胃动素(MD=32.17,95%CI:19.87~44.47,P<0.00001)、胃泌素(MD=2.69,95%CI:1.6~3.79,P<0.00001)、CGRP(MD=5.77,95%CI:4.37~7.17,P<0.00001)水平,降低ET(MD=-8.27,95%CI:-11.12~-5.42,P<0.00001)、EGF(MD=-0.92,95%CI:-1.23~-0.61,P<0.00001)水平,且不良反应方面优于未使用参芪健胃颗粒的对照组。结论参芪健胃颗粒治疗慢性萎缩性胃炎可显著提高疗效,改善中医证候,改善胃肠激素水平,提高幽门螺杆菌治愈率,且安全性良好。Objective To systematically evaluate the efficacy and safety of Shenqi Jianwei Granules in treating chronic atrophic gastritis(CAG).Methods Chinese and English databases were searched by computer,and the search time was from the establishment of each database to November 2020.The randomized controlled study(RCT)of Shenqi Jianwei Granule in treating CAG was included,and the methodological quality of the included literatures was evaluated,and meta-analysis was carried out by RevMan5.4.1 software.Results A total of 10 RCTs were included,including 887 CAG patients.Meta-analysis showed that Shenqi Jianwei Granules could improve the total effective rate of CAG[OR=4.04,95%CI(2.67~6.11),P<0.00001]and Hp(OR=2.39,95%CI:1.47~3.89,P<0.001),improve the traditional Chinese medicine syndrome[Epigastric pain(MD=-0.67,95%CI:-0.85~-0.49,P<0.00001),Gastric distention(MD=-0.59,95%CI:-0.79~-0.39,P<0.00001),Fatigue(MD=-0.67,95%CI:-0.77~-0.56,P<0.00001),anorexia(MD=-0.76,95%CI:-0.90~-0.62,P<0.00001)],increased motilin(MD=32.17,95%CI:19.87~44.47,P<0.00001),gastrin(MD=2.69,95%CI:1.6~3.79,P<0.00001)and CGRP(MD=5.77,95%CI:4.37~7.17,P<0.00001),the level of ET(MD=-8.27,95%CI:-11.12~-5.42,P<0.00001)and EGF(MD=-0.92,95%CI:-1.23~-0.61,P<0.00001)were decreased,and the study group is better than the control group without Shenqi Jianwei Granules in ADR.Conclusion Shenqi Jianwei Granules can significantly improve the curative effect of chronic atrophic gastritis,improve TCM syndrome,gastrointestinal hormone level and the cure rate of Helicobacter pylori,and the security is good.
