摘 要:目的探讨基于信息化平台进行全院血糖管理的临床应用模式与效果,以提高全院血糖管理的有效性,旨在为临床采用新型管理方式提高效果提供依据。方法苏州科技城医院建立以数据化和信息化为基础的血糖监测管理模式,实施以内分泌科医生和护士为主导,对全院住院糖尿病患者进行专业血糖管理。江苏盛泽医院进行常规会诊模式管理。通过分析住院糖尿病患者管理前后血糖数据,评价两种管理模式血糖管理效果。结果2018年1月1日至12月31日,苏州科技城医院应用信息化平台全院共管理非内分泌科住院的500例2型糖尿病患者,平均年龄(64.1±11.9)岁;平均住院时间(13.6±10.2)d;入院时HbA1c(8.65±1.75)%。与入院时比较,出院时空腹血糖下降(2.0±3.6)mmol/L,早餐后2 h血糖下降(2.7±5.0)mmol/L。江苏盛泽医院在传统普通会诊模式下管理的156例非内分泌科住院糖尿病患者平均年龄(66.7±13.3)岁;平均住院时间(13.8±8.6)d;入院时HbA1c(8.8±2.6)%。与入院时比较,出院时空腹血糖下降(0.8±3.1)mmol/L,早餐后2 h血糖下降(1.6±5.8)mmol/L;基于信息化平台下血糖管理效果优于传统内分泌科会诊模式。结论与传统会诊模式相比,基于信息化平台下全院血糖管理进一步降低了住院期间血糖水平,提高了血糖管理的效果。Objective To investigate the clinical application mode and effects of hospital-wide blood glucose management based on the information platform,so as to provide a basis for improving the effect of blood glucose management with new management methods in clinical practice.Methods A blood glucose monitoring and management model was established in Suzhou Science and Technology City Hospital based on data and information,and professional blood glucose management for inpatients with diabetes in the hospital led by endocrinologists and nurses was implemented.Regular consultation mode management was provided by Jiangsu Shengze Hospital.By analyzing the blood glucose data before and after the management of diabetic inpatients,the blood glucose management effect of the two management modes was evaluated.Results The Suzhou Science and Technology City Hospital applied the information platform to manage 500 inpatients with type 2 diabetes in Department of Non-endocrinology from January 1 to December 31,2018,with patient’s average age being(64.1±11.9)years,average length of hospital stay being(13.6±10.2)d and HbA1c(8.65±1.75)%at admission.Compared with those at admission,the levels of fasting blood glucose(FBG)decreased and 2-hour postprandial blood glucose(2hPBG)were decreased at discharge.Another 156 inpatients with diabetes under the management of the traditional general consultation mode had an average age of(66.7±13.3)years,with an average length of hospital stay being(13.8±8.6)d and HbA1c%(8.8±2.6)%at admission.Compared with those at admission,thelevels of FBG and 2hPBG were decreased at discharge.The effects of blood glucose management based on the information platform were superior to those of traditional consultation model.Conclusion Compared with the traditional consultation model,the in-hospital blood glucose management based on the information platform further reduces the blood glucose level during hospitalization and improves the effect of blood glucose management.
