固本益髓灸法联合超声引导下内热针治疗老年骨质疏松性椎体压缩骨折腰背痛的临床观察

摘　　要：目的观察固本益髓灸法联合超声引导下内热针治疗老年骨质疏松性椎体压缩骨折(Osteoporotic vertebral compression fracture,OVCF)腰背痛的临床疗效。方法将2019年5月—2020年5月期间中国人民解放军陆军第八十一集团军医院收治的120例老年OVCF腰背痛患者作为研究对象,随机分为试验组与对照组,每组60例。对照组给予伊班膦酸钠注射液静脉滴注联合碳酸钙D3咀嚼片治疗,试验组在对照组用药基础上增加固本益髓灸法联合超声引导下内热针治疗。观察两组患者临床疗效,对比两组治疗前、治疗1周、治疗3个月和治疗6个月的视觉模拟评分法(Visual analogue scale,VAS)、匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)、Oswestry功能障碍指数(Oswestrydabilityindex,ODI)和骨密度指标[血清骨碱性磷酸酶(Bone alkaline phosphatase,BALP)和骨钙素(Osteocalcin Bone Gama-Gla Protein,BGP)],并统计不良反应发生情况。结果治疗后,试验组治疗总有效率为95.00%(57/60),显著高于对照组76.67%(46/60),两组比较,差异有统计学意义(P<0.05)。与治疗前比较,两组患者治疗1周、治疗3个月和治疗6个月后VAS、PSQI和ODI评分均明显降低(P<0.05),且试验组治疗后各时间点VAS、PSQI和ODI评分均明显低于同时间点的对照组(P<0.05)。两组患者治疗3、6个月后血清BALP和BGP均较治疗前明显升高(P<0.05),试验组治疗3、6个月后血清BALP和BGP水平均明显高于同时间点的对照组(P<0.05)。治疗期间,两组患者均未见肝、肾功能损伤,皮肤灼伤,神经根损伤等不良反应。结论在常规西药用药基础上增加固本益髓灸法联合超声引导下内热针治疗老年OVCF腰背痛,可有效减轻患者疼痛,改善患者睡眠质量和骨代谢,提高临床疗效。Objective To observe the clinical effect of GubenYisui moxibustion combined with ultrasound-guided internal heat acupuncture in the treatment of low back pain in elderly patients with osteoporotic vertebral compression fracture(OVCF).Methods From May 2019 to May 2020,120 elderly OVCF patients with low back pain were admitted to the 81 st Army Hospital of the PLA were selectedas the research object.They were randomly divided into experimental group and control group,group 60 cases.The control group was given intravenous drip of ibandronate sodium injection combined with calcium carbonate D3 chewable tablets,and the experimental group was treated with medicine based on the control group+GubenYisui moxibustion combined with ultrasound-guided internal heating needle treatment.Statistics of the clinical efficacy of the two groups,and compare the visual analog pain score(VAS),Pittsburgh Sleep Quality Index(PSQI),and Oswestry Disability Index(ODI score)of the two groups before treatment,1 week of treatment,3 months of treatment,and 6 months of treatment.And bone mineral density index[serum bone alkaline phosphatase(BALP)and osteocalcin(BGP)],and statistics of adverse reactions were collected.Results After treatment,the total effective rate of the experimental group was 95.00%(57/60),which was significantly higher than 76.67%(46/60)of the control group,and the difference was statistically significant(P<0.05).Compared with before treatment,the VAS,PSQI and ODI scores of the two groups were significantly decreased after 1 week,3 months and 6 months of treatment(P<0.05),and the VAS,PSQI and ODI scores of the experimental group at each time point after treatment were significantly decreased(P<0.05).The scores were significantly lower than those of the control group at the same time point(P<0.05).After 3 and 6 months of treatment,serum BALP and BGP in the two groups were significantly higher than those before treatment(P<0.05).The serum BALP and BGP levels in the experimental group after 3 and 6 months of treatment were signi
