以童趣化理念为导向的整体护理对急性淋巴细胞白血病患儿化疗依从性、疲乏程度及不适反应的影响

摘　　要：目的:研究以童趣化理念为导向的整体护理对急性淋巴细胞白血病(ALL)患儿治疗依从性、疲乏程度和不适反应的影响。方法:利用随机数表法将深圳市儿童医院2018年10月-2020年1月收治的116例进行化疗的ALL患儿分为对照组与研究组,每组58例,分别实施儿童常规化疗护理和以童趣理念为导向的整体化疗护理。比较护理前后两组依从性和不适反应变化情况,并利用儿童多维疲乏量表(PedsQLTMMFS)对患儿护理前后的化疗疲乏程度进行评估。结果:研究组对治疗的总依从率为91.38%,明显高于对照组的77.59%(P<0.05)。研究组护理后的PedsQLTMMFS休息、认知和一般疲乏评分均低于对照组(P<0.05)。研究组纳差和负性情绪不适反应的发生率低于对照组(P<0.05),恶心呕吐、乏力、发热的发生率与对照组比较,差异无统计学意义(P>0.05)。结论:以童趣化理念为导向的整体护理可帮助提高ALL患儿化疗依从性,减轻其化疗疲乏状态及相关不适反应。Objective:To study the effect of childlike concept-oriented holistic nursing on treatment compliance,fatigue and adverse reactions in children with acute lymphoblastic leukemia (ALL).Method:Totally 116 children with ALL who underwent chemotherapy in Shenzhen Children’s Hospital form October 2018 to January 2020 were divided into the control group and the study group by random number table method,with 58 patients in each group.The control group received routine nursing and the study group received childlike concept-oriented holistic nursing.Compliance and adverse reactions in the two groups were compared,and the the pediatric quality of life inventory multidimensional fatigue scale (PedsQLTMMFS) was used to assess fatigue degree before and after nursing.Result:The totally treatment compliance rate in the study group was 91.38%,significantly higher than 77.59% in the control group (P<0.05).After nursing,the study group had lower PedsQLTMMFS scores than the control group in terms of rest,cognition and general fatigue (P<0.05).The incidences of anorexia and negative emotions in the study group were lower than those in the control group (P<0.05),but the incidences of nausea and vomiting,fatigue and fever showed no statistically significant differences between the two groups (P>0.05).Conclusion:Childlike concept-oriented holistic nursing can help improve the chemotherapy compliance of children with ALL and reduce related adverse reactions.
