摘 要:目的:探讨指动脉静脉化在末节断指再植中的应用及其效果。方法:选取遵义医科大学第五附属(珠海)医院2017年3月-2020年7月42例(42指)接受末节断指再植手术患者。按照入院先后顺序将所有患者分成甲组和乙组,每组21例。甲组给予常规单纯静脉吻合治疗,乙组给予指动脉静脉化处理。对比两组术后断指再植恢复情况、术后相关指标及并发症发生率。结果:术后甲组断指再植成活率与一期愈合率分别为71.43%、61.90%,均显著低于乙组的95.24%、90.48%(P<0.05);术后6个月,乙组断指再植术后指甲长度长于甲组,远端指间关节活动度显著大于甲组,两点辨别觉则显著短于甲组,差异均有统计学意义(P<0.05);甲组术后并发症发生率显著高于乙组(P<0.05)。结论:针对临床末节断指再植手术患者情况,术中给予其指动脉静脉化处理,可明显提高患者断指成活率与一期愈合率,同时对改善患者断指关节活动度、降低术后并发症发生率均有着重要作用,整体效果显著,值得临床大力推广应用。Objective:To explore the application and effect of digital artery venous in replantation of distal severed finger.Method:A total of 42 patients (42 fingers) received replantation surgery of distal severed finger were selected from the Fifth Affiliated (Zhuhai) Hospital of Zunyi Medical University from March 2017 to July 2020.All patients were divided into group A and group B according to the order of admission,with 21 patients in each group.Group A was treated with conventional venous anastomosis,and group B was treated with digital artery venous.The recovery of replantation of severed finger after surgery,postoperative related indicators and incidence of complications were compared between the two groups.Result:The survival rate and primary healing rate of severed finger replantation in group A were 71.43% and 61.90% respectively,which were significantly lower than 95.24% and 90.48% in group B (P<0.05);six months after surgery,the nail length after severed finger replantation in group B was longer than that in group A,and the range of motion of the distal interphalangeal joint in group B was significantly greater than that in group A,while the two-point discrimination was significantly shorter than that in group A,the differences were statistically significant (P<0.05);the incidence of postoperative complications in group A was significantly higher than that in group B (P<0.05).Conclusion:According to the clinical situation of patients undergoing replantation surgery of distal severed fingers,the handling method of digital artery venous given for them in intraoperative can significantly increase the survival rate of severed fingers and primary healing rate.At the same time,it plays an important role in improving the range of motion of patients’ severed finger joints and reducing the incidence of postoperative complications.The overall effect is significant,which is worthy of clinical promotion and application.
