摘　　要：目的:探讨用鼻内镜下低温等离子消融术治疗鼻咽纤维血管瘤的效果。方法:选择2020年1月至12月期间绵阳市中心医院收治的80例鼻咽纤维血管瘤患者作为研究对象。按照随机数表法将其分为电凝术组和消融术组,每组各有患者40例。对电凝术组患者进行传统的鼻内镜下电刀、单/双极电凝分离术,对消融术组患者进行鼻内镜下低温等离子消融术,然后比较两组患者的各项临床指标。结果:消融术组患者术中的出血量少于电凝术组患者,其手术的时间、术后住院的时间和术后鼻咽黏膜上皮化的时间均短于电凝术组患者,差异有统计学意义(P<0.05)。术后3 d、5 d、7 d及14 d,消融术组患者视觉模拟评分法(VAS)的评分均低于电凝术组患者,差异有统计学意义(P<0.05)。结论:用鼻内镜下低温等离子消融术治疗鼻咽纤维血管瘤的效果显著,具有术中的出血量少、手术的时间和患者术后恢复的时间短及术后疼痛轻等优点。Objective:To investigate the effect of low-temperature plasma ablation under nasal endoscope in the treatment of nasopharyngeal fibroangioma.Methods:A total of 80 patients with nasopharyngeal fibrohemangioma admitted to Mianyang Central Hospital from January to December 2020 were selected as the research object.They were divided into electrocoagulation group and ablation group according to random number table method,with 40 patients in each group.The patients in the electrocoagulation group were treated with traditional endoscopic electrotomy and single/bipolar electrocoagulation separation,and the patients in the ablation group were treated with endoscopic low-temperature plasma ablation,and then the clinical indicators of the two groups were compared.Results:The amount of intraoperative blood loss in the ablation group was less than that in the electrocoagulation group,and the operation time,postoperative hospitalization time and postoperative epithelial time of nasopharyngeal mucosa were shorter than those in the electrocoagulation group,with statistical significance(P<0.05).The visual analog scale(VAS)scores of patients in ablation group were lower than those in electrocoagulation group at 3 d,5 d,7 d and 14 d after surgery,with statistical significance(P<0.05).Conclusion:Low-temperature plasma ablation under nasal endoscope is effective in treating nasopharyngeal fibrohemangioma,which has the advantages of less intraoperative blood loss,short operation time and postoperative recovery time,and light postoperative pain.
