摘 要:目的:探讨肾小球滤过率(GFR)估算公式在慢性肾脏病(CKD)患者肾功能受损程度评估中的应用价值。方法:选取南京医科大学附属南京医院收治的264例CKD患者作为研究对象。以99mTc-DTPA肾动态显像法测定其GFR估算公式,分析各GFR估算公式(CG公式、MDRD公式、CKD-EPI公式、FAS公式)在CKD患者肾功能受损程度评估中的应用价值。结果:各GFR估算公式算得的GFR与GFR实际值显著相关(P<0.05),其中CG公式算得的GFR与GFR实际值的相关性最高,为0.937。就测量值分析,各GFR估算公式算得的结果均低于GFR的实际值,其中MDRD公式算得的结果与GFR的实际值最为接近,平均偏差为0.541,二者相比差异无统计学意义(P>0.05)。其余GFR估算公式算得的结果与GFR的实际值相比,差异均有统计学意义(P<0.05)。CKD分期与各GFR估算公式算得的GFR相关,且在CKD3期GFR相对较高,但在CKD5期无明显相关性(P>0.05)。结论:进行99mTc-DTPA肾动态显像检查时,以不同公式计算GFR的结果存在差异,临床上可依据患者的实际情况选择合适的GFR估算公式,以便早期发现CKD患者肾功能受损的程度,为此病的早期诊断和治疗提供依据。Objective:To explore the application value of glomerular filtration rate(GFR)estimation formula in the evaluation of renal function impairment in patients with chronic kidney disease(CKD).Methods:A total of 264 patients with CKD in Nanjing Hospital affiliated to Nanjing Medical University were selected as subjects.The GFR estimation formulas(CG formula,MDRD formula,CKD-EPI formula and FAS formula)were determined by 99MTC-DTPA renal dynamic imaging,and the application value of each GFR estimation formula(CG formula,MDRD formula,CKD-EPI formula and FAS formula)in the evaluation of renal function impairment in PATIENTS with CKD was analyzed.Results:The GFR calculated by each GFR estimation formula was significantly correlated with the actual VALUE of GFR(P<0.05),and the CORRELATION between THE GFR calculated by CG formula and the actual value of GFR was the highest(0.937).On the analysis of measured values,the calculated results of each GFR estimation formula were all lower than the actual value of GFR,among which the calculated results of MDRD formula were the closest to the actual value of GFR,with an average deviation of 0.541,and there was no statistical significance between them(P>0.05).Compared with the actual value of GFR,the results calculated by other GFR estimation formulas showed statistically significant differences(P<0.05).CKD stage was correlated with GFR calculated by each GFR estimation formula,and GFR was relatively higher in CKD3 stage,but no significant correlation was found in CKD5 stage(P>0.05).Conclusions:There are differences in the results of calculating GFR with different formulas during 99MTC-DTPA renal dynamic imaging.In clinical practice,appropriate GFR estimation formula can be selected according to the actual situation of patients,so as to find the degree of renal impairment in patients with CKD at an early stage and provide a basis for the early diagnosis and treatment of CKD.
