摘　　要：目的探讨心内科冠心病合并肺部感染患者血清降钙素原(PCT)、C-反应蛋白(CRP)、白细胞介素-6(IL-6)水平变化及临床意义。方法回顾性分析2018年3月至2021年3月在广东省潮州市中心医院(以下简称“我院”)就诊的80例冠心病患者的诊疗情况,根据肺部感染诊断标准将其分为感染组(32例)和非感染组(48例),另选择同期在我院体检的健康人群50例为对照组。检测比较各组血清PCT、CRP、IL-6水平,并根据Gensini评分评估感染组冠脉病变程度,采用Pearson直线相关分析PCT、CRP、IL-6水平与Gensini评分的相关性。结果感染组和非感染组血清PCT、CRP、IL-6水平明显高于对照组,感染组血清PCT、CRP、IL-6水平高于非感染组,差异有统计学意义(P<0.05)。两组Gensini评分比较,差异有高度统计学意义(P<0.01)。Pearson直线相关分析显示,感染组患者血清PCT(r=0.603)、CRP(r=0.685)、IL-6(r=0.574)水平与Gensini评分呈正相关(P<0.01)。结论心内科冠心病合并肺部感染患者血清PCT、CRP、IL-6水平明显升高,以上指标的检测对冠心病合并肺部感染具有一定的诊断价值。Objective To investigate the changes and clinical significance of serum procalcitonin(PCT),C-reactive protein(CRP),and interlukin-6(IL-6)levels in patients with coronary heart disease complicated with pulmonary infection in department of cardiology.Methods The diagnosis and treatment situation of 80 patients with coronary heart disease who were treated in Chaozhou Central Hospital in Guangdong Province(“our hospital”for short)from March 2018 to March 2021 were retrospectively analyzed.According to the diagnostic criteria of pulmonary infection,they were divided into infection group(32 cases)and non-infection group(48 cases),and 50 healthy people who underwent physical examination in our hospital during the same period were selected as control group.Serum levels of PCT,CRP,and IL-6 in each group were detected and compared,and the degree of coronary lesions in the infection group was evaluated according to Gensini score.Pearson linear correlation was used to analyze the correlation between PCT,CRP,IL-6 levels,and Gensini score.Results The serum levels of PCT,CRP,and IL-6 in the infection group and non-infection group were significantly higher than those in the control group,while the serum PCT,CRP,and IL-6 levels in the infection group were higher than those in the non-infection group,and the differences were statistically significant(P<0.05).There was a statistically significant difference in Gensini score between the two groups(P<0.01).Pearson linear correlation analysis showed that the levels of serum PCT(r=0.603),CRP(r=0.685),IL-6(r=0.574)in the infection group were positively correlated with Gensini scores(P<0.01).Conclusion The serum levels of PCT,CRP,and IL-6 in patients with coronary heart disease complicated with pulmonary infection in the department of cardiology are significantly increased.The detection of the above indicators has certain diagnostic value for coronary heart disease complicated with pulmonary infection.
