摘 要:目的:观察针刀干预对膝骨关节炎(KOA)兔股直肌Wnt3a/β-catenin信号通路调控作用及纤维化的影响,探讨针刀治疗KOA的可能作用机制。方法:24只健康雄性新西兰兔随机分为空白组、模型组、针刀组,每组8只。改良Videman法左后肢伸直位石膏固定制动6周复制KOA模型,Lequesne MG膝关节级别评分、X线和MRI确认造模成功后,采用经筋理论指导下在股四头肌鹤顶次、髌外上、髌内上经筋病灶点上行针刀松解治疗,每周1次,共治疗4次。采用Masson染色观察股直肌纤维化改变,Real-time PCR和Western Blot检测股直肌Wnt3a、β-catenin、TGF-β、CTGF、ColⅠmRNA和蛋白表达水平。结果:与空白组比较,模型组股直肌纤维化明显,胶原容积分数显著增加(P<0.05),Wnt3a、β-catenin、TGF-β、CTGF、ColⅠmRNA及蛋白表达水平显著升高(P<0.05);与模型组比较,针刀组股直肌纤维化减轻,胶原容积分数显著减小(P<0.05),Wnt3a、β-catenin、TGF-β、CTGF、ColⅠmRNA及蛋白表达水平显著降低(P<0.05)。结论:基于经筋理论针刀松解股四头肌经筋病灶点能通过调节Wnt3a/β-catenin信号通路,减轻KOA兔股直肌纤维化程度,这可能是针刀治疗KOA的作用机制之一。Objective:To observe the effect of acupotomy intervention on the regulation of Wnt3a/β-catenin signaling pathway and fibrosis in the rectus femora of rabbits with knee osteoarthritis(KOA),and to explore the possible mechanism of acupotomy in the treatment of KOA.Methods:Healthy male New Zealand rabbits were randomly divided into blank group,model group and acupotomy group,with 8 rabbits in each group.Modified Videman method was used to replicate the osteoarthritis model of the knee with left hind limb extended plaster immobility for 6 weeks.The blank group and model group were routinely fed and grabbed without intervention.In acupotomy group,the transfascial focal points of quadriceps femoris muscle were released by acupotomy under the guidance of jingjin theory,treatment points include Hedingci,Binwaishang,Binneishang.Once a week,a total of 4 times of treatment.Rectus femoris was taken and Masson staining was used to observe the morphological changes of rectus femoris.Real-time PCR and Western Blot was used to detect the mRNA and protein expression levels of Wnt3a,β-catenin,TGF-β,CTGF,Col I.Results:Compared with the blank group,the fibroification of the rectus femoris and the volume fraction of collagen were significantly increased in the model group(P<0.05).The mRNA and protein expression levels of Wnt3a,β-catenin,TGF-β,CTGF,Col I were significantly increased(P<0.05).Compared with the model group,the fibrillation of the rectus femoris was reduced and the volume fraction of collagen was significantly reduced in the acupotomy group(P<0.05).The mRNA and protein expression levels of Wnt3a,β-catenin,TGF-β,CTGF,Col I were significantly reduced(P<0.05).Conclusion:Based on jingjin theory,the release of quadriceps femoris by acupotomy can regulate the Wnt3a/β-catenin signaling pathway,to reduce the degree of femur rectus fibrosus in rabbits with KOA.This may be one of the mechanisms of acupotomy in treating KOA.
