基于情绪轴及性腺轴探讨疏肝补肾法治疗乳腺增生症的机制

摘　　要：目的:揭示疏肝法、补肾法、疏肝补肾法治疗乳腺增生症(MGH)的疗效,并从情绪轴和性腺轴的改善探讨不同治则治法的作用机制及协同机制。方法:Wistar大鼠随机分为空白对照组、模型组、西药组、疏肝组、补肾组、疏肝补肾组。除空白对照组大鼠外,其他各组造成MGH模型,空白对照组和模型组以0.9%氯化钠溶液灌胃,其余各组以等效剂量灌服相应药物。用药第28天后测量大鼠乳头体积、病理形态、乳腺增殖细胞核抗原(PCNA)表达情况、不同激素水平的变化。结果:与模型组比较,补肾组和疏肝补肾组大鼠乳头体积显著降低(P<0.05)。从病理而言,补肾组与疏肝补肾组改善最为明显。补肾组与疏肝补肾组PCNA阳性细胞数少于模型组。与模型组比较,疏肝组皮质醇含量显著降低(P<0.05)。结论:补肾法在改善MGH形态学上更具优势,疏肝法调节应激状态效果明显,疏肝、补肾法协同机制并非体现在对一种症状的叠加增效上,而体现在对MGH不同症状的调节效果上。Objective:To reveal the curative effect of soothing liver,nourishing kidney and soothing liver plus nourishing kidney in the treatment of mammary gland hyperplasia(MGH)by improving hypothalamic-pituitary-adrenal axis(HPA)and hypothalamic-pituitary-ovary axis(HPO).Methods:Wistar rats were randomly divided into normal group,model group,western medicine group,soothing liver group,nourishing kidney group,soothing liver plus nourishing kidney group.In addition to the normal group,the other groups were built up the rat model of the MGH.The rats in normal and model group were given normal saline,the other groups were given corresponding drugs at equivalent dose.The volume of nipple,pathological morphology,proliferating cell nuclear antigen(PCNA)in mammary gland,and changes of different hormone levels were measured after 28 days of treatment.Results:Compared with the model group,the nipple volume of the nourishing kidney group and soothing liver plus nourishing kidney group was significantly decreased(P<0.05).In terms of pathology,the improvement was most obvious in the nourishing kidney group and soothing liver plus nourishing kidney group,the number of PCNA positive cells in the nourishing kidney group and soothing liver plus nourishing kidney group was lower than that in the model group.Compared with the model group,the cortisol of the soothing liver group was significantly decreased(P<0.05).Conclusion:The method of nourishing kidney is more advantageous in improving the morphology of MGH.The method of soothing liver has obvious effect on regulating stress state.The synergistic mechanism of soothing liver and nourishing kidney is not reflected in the superposition effect of one symptom,but in the regulation effect of different symptoms of MGH.
