摘　　要：子宫内膜异位症(EM)最典型的症状为继发性痛经并伴进行性加重,其发病机制尚未完全明确,目前主要认为与子宫内膜种植学说、上皮化生学说、诱导学说等有关。西医主要采用非甾体止痛药、口服避孕药和促性腺激素释放激素激动剂(GnRH-a)进行治疗,中医将其归属于"经行腹痛"范畴,龙江韩氏妇科认为气为血之帅,血随气生,亦赖气行,而人体郁、寒、热、虚皆犯于气,致气机受阻,血行不畅,血脉瘀阻不通,不通则痛,发生EM痛经。血瘀为发病之标,而气的失常贯穿致瘀的始终,为发病之本。从郁、寒、热、虚终致血瘀而痛的四点病因切入,并运用调气化瘀法论治,临床上屡获良效。文章附临床医案一则,以供同道探讨。The most typical symptom of endometriosis(EM)is secondary dysmenorrhea accompanied by progressive aggravation,and its pathogenesis has not been completely clear.At present,it is mainly thought to be related to the theory of endometrial implantation,the theory of epithelial metaplasia,the theory of induction,and so on.In western medicine,nonsteroidal analgesics,oral contraceptives and gonadotropin releasing hormone agonists(GnRH-a)are mainly used.Traditional Chinese medicine ascribes this disease to the category of meridians and abdominal pain.Longjiang HANs’gynaecology thinks that qi is the commander of blood.Blood is generated with qi and depends on qi.Qi is responsible for depression,cold,heat and deficiency of human body.Qi is blocked,blood flow is blocked,blood stasis is blocked,and pain occurs when it is blocked.Blood stasis is the mark of the disease,and the disorder of qi runs through the whole process of causing blood stasis,which is the root of the disease.From the four causes of blood stasis and pain caused by depression,cold,heat and deficiency,the treatment is treated by regulating qi and removing blood stasis.Good results were obtained in clinic.A clinical case is attached for discussion.
