经胸超声心动图在先天性心脏病复合畸形患者行同期介入治疗中的应用

摘　　要：目的分析经胸超声心动图(TTE)在先天性心脏病(CHD)复合畸形患者行同期介入治疗中的应用效果。方法选取CHD复合畸形患者118例为研究对象,并根据手术方法不同将患者分为介入治疗组26例和常规手术组92例。比较两组患者的围手术期指标和并发症发生率,随访介入治疗组的临床疗效。结果介入治疗组患者各项围手术期指标及术后并发症发生率均小于常规手术组,术后6个月左室每搏量(LVSV)大于常规手术组、左室射血分数(LVEF)小于常规手术组,差异均有统计学意义(P<0.05);介入治疗组患者术后6个月无残余分流占92.31%(24/26),ASD+VSD、VSD+PDA患者左室舒张末期内径(LVDd)较术前明显减小,ASD+VSD、ASD+PDA患者右心室内径(RVD)较术前明显减小,差异均有统计学意义(P<0.05)。结论TTE引导下同期介入治疗CHD复合畸形患者安全、有效,具有无辐射、手术创伤小、并发症发生率低等优点,有一定的临床应用前景。Objective To analyze the application effect of transthoracic echocardiography(TTE)in simultaneous percutaneous intervention in patients with congenital heart disease(CHD)complicated with compound deformities.Methods 118 patients with CHD complicated with compound deformities in our hospital were selected and divided into interventional therapy group(n=26)and routine operation group(n=92)according to different surgical methods.The perioperative indicators and incidence of complications were compared between the two groups,and the clinical efficacy of the interventional therapy group was followed up.Results Various perioperative indicators and the incidence of postoperative complications in the interventional treatment group were lower than those in the routine operation group,6 months after operation,LVSV was higher than conventional surgery group,LVEF was lower than conventional surgery group,the differences were statistically significant(P<0.05).92.31%(24/26)of the patients in the interventional therapy group had no residual shunt 6 months after operation.The LVDd was significantly decreased in ASD+VSD and VSD+PDA patients.The RVD was significantly decreased in ASD+VSD and ASD+PDA patients.The differences were statistically significant(P<0.05).Conclusions The simultaneous percutaneous intervention of CHD complicated with compound deformities under the guidance of TTE is safe and effective,with the advantages of non-radiation,little surgical trauma and the incidence of complications,and has a certain prospect of clinical application.
