摘  要:目的探讨两种二维斑点追踪成像(2D-STI)软件评价健康年轻人左心室整体纵向峰值应变(GLPS)及峰值应变离散度(PSD)的临床价值。方法收集10~24岁正常年轻人155例,按年龄分为3组:1组40例(10~14岁),2组79例(15~19岁),3组36例(20~24岁);使用2DStrain和AFI软件分别测量左室GLPS和PSD并比较两种方法测量差异,分析PSD的相关性因素。结果1组GLPS绝对值均大于其他2组(P<0.05),而PSD值在各组间无统计学差异(P>0.05)。AFI软件获得的PSD_(AFI)大于PSD_(2DStrain)(P<0.05),两种软件的GLPS差异无统计学意义(P>0.05);PSD与GLPS绝对值呈负相关(r=-0.328,P<0.001);GLPS和PSD测值在观察者内及观察者间重复性良好。结论两种2D-STI分析软件均可准确、快捷地评价年轻人左室收缩功能和同步性,但应用PSD评估心肌收缩同步性时需注意方法学差异。Objective To explore the value of method-specific left ventricular(LV)global longitudinal peak strain(GLPS)and longitudinal peak strain dispersion(PSD)derived from two-dimensional speckle tracking imaging(2 D-STI)in a healthy young cohort.Methods A total of 155 healthy young people aged 10-24 years were collected and further divided into three groups according to age:group 1 with 40 cases(10-14 years old),group 2 with 79 cases(15-19 years old)and group 3 with 36 cases(20-24 years old).Method-specific values of GLPS and PSD were measured by 2 DStrain and AFI software in each group,and the differences between the two methods were compared respectively correlation factors of PSD were also andlyzed.Results The absolute values of GLPS in the youngest group were significantly higher than those in the other two groups(P<0.05),while no significant difference in PSD was shown among the groups(P>0.05).Obviously,PSD obtained by AFI software was significantly higher than that by 2 DStrain software(P<0.05),whereas the difference of GLPS between the two software yielded no significance(P>0.05).Moreover,PSD was negatively correlated with the absolute value of GLPS(r=-0.328,P<0.001).GLPS and PSD had good repeatability within and between observers.Conclusions Both 2 D-STI analysis software could accurately and efficiently evaluate LV performance and synchrony in the healthy young people,and method-specific values of PSD should be considered.
