盆底超声对腹腔镜阴道骶骨固定术后的观察及疗效评估

摘　　要：目的探讨盆底超声对盆腔器官脱垂患者行腹腔镜阴道骶骨固定术后植入网片的观察及疗效评估的价值。方法对34例行腹腔镜阴道骶骨固定术的患者于其术后3~36个月进行复查,按临床疗效的差异进行分组,比较手术前后患者的盆底结构和功能的差异,观察植入网片的悬吊及活动情况,并测量相关参数。结果被检患者中,治愈23例,术后复发11例。治愈组在术后行Valsalva动作时膀胱最低点、阴道顶端、直肠壶腹部的下降位置均较复发组明显上升,阴道顶端下移距离较复发组明显减小,肛提肌裂孔面积较复发组明显缩小,差异有统计学意义(P<0.05)。结论盆底超声能清晰显示腹腔镜阴道骶骨固定术植入网片的位置、形态及活动情况,并为患者术后的疗效评估提供有价值的影像学依据。Objective To investigate the value of pelvic floor ultrasonography in the observation and evaluation of implanted mesh after laparoscopic sacrocolpopexy in patients with pelvic organ prolapse.Methods 34 patients who underwent laparoscopic sacrocolpopexy were re-examination 3 to 36 months after the surgery.The patients were devided into group according to the clinical efficacy,and the differences in the structure and function of the pelvic floor before and after the operation were compared.The suspension and activity of the implanted mesh were observed,and related parameters were measured.Results Among the examined patients,23 cases were cured and 11 cases recurred of pelvic organ prolapse after the operation.In Valsalva operation,the lower position of the lowest point of the bladder,the apical vaginal and rectal ampulla were all significantly increased than those in recurrence group,the lower distance of the apical vaginal was significantly decreased than that in the recurrence group,the area of Levator hiatus was significantly smaller than that of the recurrence group,and the difference was statistically significant(P<0.05).Conclusions Pelvic floor ultrasonography can clearly show the position,shape and activity of the laparoscopic sacrocolpopexy implanted mesh and provide valuable imaging evidence for the postoperative curative effect evaluation.
