摘  要:目的多中心研究构建安徽省成人骨骼肌量下降的诊断切点。方法首先验证生物电阻抗分析仪(bioelectrical impedance analyzer,BIA)和双能X射线吸收仪(dual energy X-ray absorptiometry,DEXA)在计算骨骼肌量上的一致性。在安徽省合肥市、蚌埠市、滁州市3家医院招募进行健康检查的7371名成人使用BIA测量体成分,分析成人骨骼肌量变化规律并计算四肢骨骼肌质量指数(adult skeletal muscle mass and calculate skeletal muscle mass index,ASMI)。以18~<40岁年轻成人的平均ASMI减2倍标准差,≥60岁老年人最小五分位数的ASMI,2种方法分性别构建安徽省成人骨骼肌量下降的诊断切点。结果BIA和DEXA在计算骨骼肌量上一致性良好,安徽省男性和女性的ASMI均在40~<50岁达到峰值,之后随增龄而逐渐下降。采用年轻成人骨骼肌量数据构建的诊断切点为男性ASMI<6.6 kg/m^(2),女性ASMI<5.0 kg/m^(2),使用此切点筛查肌量下降的检出率为1.22%;采用老年人骨骼肌量数据构建的诊断切点为男性ASMI<6.5 kg/m^(2),女性ASMI<5.4 kg/m^(2),使用此切点筛查肌量下降的检出率为2.66%。这2种方法构建的骨骼肌量下降诊断切点均低于中国肌少症专家共识(草案)的切点(肌量下降检出率为9.10%)。结论本研究构建的安徽省成人骨骼肌量下降诊断切点低于中国肌少症专家共识(草案)的切点,应基于安徽本地人群数据建立区域骨骼肌量下降诊断标准。Objective To establish the diagnostic cut-off points of adult skeletal muscle mass decline in Anhui Province by multicenter cross-sectional study.Methods Firstly,the consistency of bioelectrical impedance analyzer(BIA)and dual energy X-ray absorptiometry(DEXA)in calculating skeletal muscle mass was verified.7371 adults,who received the health examination in three hospitals in Hefei City,Bengbu City and Chuzhou City of Anhui Provine,were recruited for measuring the body composition by BIA,to analyze the variation of adult skeletal muscle mass and calculate adult skeletal muscle mass index(ASMI).The diagnostic cut-off points of the decrease of adult skeletal muscle mass in Anhui Province were constructed by using the average ASMI reduction of 2 times of standard difference in 18-<40 year old adults and the minimum quintile of ASMI in elderly over 60years old.Results There was a good consistency between BIA and DEXA in the calculation of skeletal muscle mass.The ASMI of men and women in Anhui Province reached the peak at the age of 40-<50 years old,and then gradually decreased with age.The diagnostic cut-off points constructed by skeletal muscle mass data of young adults were as follows:Male ASMI<6.6 kg/m^(2),female ASMI<5.0 kg/m^(2),and the detection rate of muscle mass decrease by this cut-off point was 1.22%.The diagnostic cut-off points constructed by skeletal muscle mass data of old people was as follows:Male ASMI<6.5 kg/m^(2),female ASMI<5.4 kg/m^(2),and the detection rate of muscle mass decrease by this cut-off point was 2.66%.The cut-off points constructed by the two methods were lower than the cut-off points of the Chinese expert consensus on sarcopenia(Draft)(the detection rate of muscle mass decrease was 9.10%).Conclusion The cut-off points of adult skeletal muscle mass decline in Anhui Province constructed in this study are lower than the cut-off points of Chinese expert consensus on sarcopenia(Draft),and the regional diagnostic criteria for skeletal muscle mass decline should be established in the basis
