摘  要:目的:探讨白子菜对2型糖尿病合并急性肾损伤小鼠(T2DM+AKI)的保护作用,并从炎症反应角度探讨其初步疗效机制。方法:取90只SPF级KM小鼠随机分为正常对照组、T2DM组、(T2DM+AKI)组、(白子菜高、低剂量+T2DM)组、(白子菜高、低剂量+T2DM+AKI)组、(二甲双胍+T2DM)组和(二甲双胍+T2DM+AKI)组,每组10只。造模小鼠喂食高糖高脂饲料结合一次性腹腔注射100mg/kg链脲佐菌素复制T2DM模型,再腹腔注射LPS(10mg/kg)复制(T2DM+AKI)模型。治疗组小鼠在腹腔注射LPS前7d开始,用白子菜水煎液灌胃,1次/d,持续15d。除正常对照组外,其余各组小鼠全程喂养高脂饲料直至实验结束。末次给药后禁食不禁水12h,检测各组小鼠空腹血糖及血清尿素氮(BUN)、血肌酐(Scr)含量;ELISA试剂盒检测肾组织中肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)和白细胞介素-10(IL-10)含量;并取部分肾脏组织做HE染色切片,观察肾组织损伤情况。结果:与正常对照组比较,T2DM组、(T2DM+AKI)组小鼠肾小球萎缩变形,数目明显减少,可见严重的肾损伤,且(T2DM+AKI)组较T2DM组严重;空腹血糖、血清BUN和Scr及肾组织TNF-α、IL-6含量明显增加,而IL-10含量明显减少,且(T2DM+AKI)组较T2DM组增加或减少更明显。与T2DM组、(T2DM+AKI)组比较,白子菜组小鼠肾小球结构清晰,数目明显增多,大小形态基本恢复正常,肾小球萎缩现象得到一定程度的缓解;空腹血糖、血清BUN和Scr及肾组织TNF-α、IL-6含量明显减少,而IL-10含量明显增加。结论:白子菜对(T2DM+AKI)具有保护作用,表现为同时降低空腹血糖、BUN和Scr含量,而其机制可能与抑制炎症反应有关。Objective:To investigate the protective effect of Gynura divaricata on type 2 diabetes mellitus with acute kidney injury(T2 DM+AKI)mice,and to explore its preliminary therapeutic mechanism from the Angle of inflammatory response.Methods:90 SPF KM mice were randomly divided into normal control group,T2 DM group,(T2 DM+AKI)group,(Gynura divaricata high and low dose+T2 DM+AKI)group,(metformin+T2 DM)group and(metformin+T2 DM+AKI)group,with 10 mice in each group.The model mice were fed high glucose and high fat diet combined with one-time intraperitoneal injection of 100 mg/kg streptozotocin to replicate the T2 DM mouse model,and then intraperitoneal injection of LPS(10 mg/kg)to replicate the T2 DM+AKI mouse model.Mice in the treatment group were intragastrically administrated with Gynura divaricata water decoction,once a day,for 15 days,starting from 7 days before intraperitoneal injection of LPS.Except normal control group,mice in other groups were fed high-fat diet throughout the experiment until the end of the experiment.Fasting blood glucose,serum urea nitrogen(BUN)and serum creatinine(SCR)were measured after fasting for 12 hours after the last administration.The levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-10(IL-10)in renal tissues were determined by ELISA kit.HE staining section was used to observe the damage of renal tissue.Results:Compared with normal control group,the number of glomerular atrophy and deformation in T2 DM group and(T2 DM+AKI)group was significantly reduced.Severe renal injury was observed in T2 DM+AKI group.The contents of fasting blood glucose,serum BUN,Scr,TNF-αand IL-6 in renal tissue were significantly increased,while the contents of IL-10 were significantly decreased,and the increase or decrease was more obvious in(T2 DM+AKI)group than in T2 DM group.Compared with T2 DM group and(T2 DM+AKI)group,the structure and number of glomeruli in the Gynura divaricata group were clear,the size and shape of glomeruli were basically normal,and the glomerular atrophy
