生地茯苓汤口服联合迪银片治疗寻常型银屑病的效果及对炎性因子、内皮素水平的影响

摘　　要：目的:研究生地茯苓汤口服联合熏洗联合迪银片治疗寻常型银屑病的效果及对炎性因子、内皮素水平的影响。方法:将于医院就诊的130例银屑病患者随机分为对照组(n=65,采用迪银片治疗)和观察组(n=65,在对照组治疗基础上口服生地茯苓汤联用熏洗),比较治疗前和治疗12周后两组患者中医症状积分、病情程度[银屑病面积与严重性指数(PASI)评分]、疼痛度[视觉模拟评分法(VAS)]评分、炎性因子[白细胞介素-4(IL-4)、白细胞介素-8(IL-8)、白细胞介素-17(IL-17)、肿瘤坏死因子(TNF-α)]、内皮素(ET)、血管内皮生长因子(VEGF)指标。结果:两组患者中医症状积分、PASI评分、VAS评分、IL-2、IL-6、IL-8、TNF-α、ET水平均较治疗前显著降低,且观察组显著低于对照组(P<0.05);两组VEGF较治疗前均显著升高,且观察组显著高于对照组(P<0.05)。结论:口服生地茯苓汤联合熏洗联合迪银片对寻常型银屑病具有显著疗效,可有效减轻疼痛并改善炎性因子、内皮素水平,可在临床广泛应用。Objective:To investigate the effects of oral administration of Shengdi Fuling Decoction combined with fumigation and washing combined with Diyin Tablet in the treatment of psoriasis vulgaris and the effects on the levels of inflammatory factors and endotheliin.Methods:130 patients with psoriasis in our hospital were randomly divided into control group(n=65,treated with Diyin Tablet)and combined group(n=65,treated with Shengdi Fuling Decoction combined with fumigation on the basis of control group).TCM symptom score,severity of disease[Psoriasis area and severity index(PASI)score],degree of pain[visual analog scale(VAS)score],inflammatory factors[interleukin-4(IL-4),interleukin-8(IL-8),interleukin-17(IL-17)]and tumor were compared between the two groups before treatment and 12 weeks after treatment Necrosis factor(TNF-α),endothelin(ET)and vascular endothelial growth factor(VEGF).Results:Compared with before treatment,TCM symptom score,PASI score,VAS score,IL-2,IL-6,IL-8,TNF-α and ET levels in 2 groups were significantly decreased,and the combined group was significantly lower than the control group(P<0.05);VEGF in both groups was significantly increased compared with before treatment,and the combined group was significantly higher than the control group(P<0.05).Conclusion:Oral Shengdi Fuling Decoction combined with fumigation and washing combined with Diyin Tablet has a significant effect on psoriasis vulgaris,which can effectively relieve pain and improve the levels of inflammatory factors and endothelin,and can be widely used in clinical practice.
