精神科药品不良反应938例分析

摘　　要：目的:探讨江西省精神病院精神科常用药品不良反应(ADR)发生的特点与规律,为临床合理用药提供参考。方法:对江西省精神病院2019年1月至2021年4月收集并上报到国家不良反应监测中心的ADR报告938例,按照精神科患者的性别与年龄、原患疾病分布、药品种类及排名前10药品、合并用药、累及系统-器官及临床表现、报告类型和转归情况等进行统计分析。结果:938例ADR报告中,19~30岁年龄段ADR发生率最高,358例(38.17%);发生ADR患者的原患疾病以精神分裂症623例(66.42%)为主;抗精神病药物发生ADR排名前三分别是利培酮233例(24.84%),氯氮平178例(18.98%),奥氮平174例(18.55%);ADR累及系统为神经系统较多,310例(33.05%)。结论:医院应加大对医务人员合理用药知识和药品不良反应识别、处理、上报等的培训力度,临床药师应继续加强临床用药指导,加强对ADR监测,降低患者ADR发生率,保障患者用药安全。Objective:To explore the characteristics and regularity of adverse drug reactions(ADR)commonly used in psychiatric department of Jiangxi Mental Health Center,so as to provide reference for clinical rational drug use.Methods:938 ADR reports collected and reported to the national adverse reaction monitoring center in Jiangxi Mental Health Center from January 2019 to April 2021,were statistically analyzed according to the gender and age of psychiatric patients,the distribution of primary diseases,drug types and top 10 drugs,combined drugs,involved systems organs and clinical manifestations,report types and outcomes.Results:Among 938 ADR reports,the incidence of ADR was the highest in the age group of 19-30 years old,358 cases(38.17%);623 cases(66.42%)of patients with ADR were mainly schizophrenia;the top three cases of ADR with antipsychotics were risperidone 233 cases(24.84%),clozapine 178 cases(18.98%)and olanzapine 174 cases(18.55%);ADR involved more nervous system,310 cases(33.05%).Conclusion:The center should strengthen the training of medical staff on rational drug use knowledge and identification,treatment and reporting of adverse drug reactions.Clinical pharmacists should continue to strengthen clinical drug use guidance,strengthen ADR monitoring,reduce the incidence of ADR and ensure the safety of drug use.
