刺血疗法联合四黄清热散外敷治疗急性痛风性关节炎的临床研究

摘　　要：目的探讨刺血疗法联合四黄清热散外敷治疗急性痛风性关节炎的临床研究。方法选取2018年8月至2021年5月于湛江市第一中医医院骨二科住院治疗的急性痛风性关节炎患者60例,按照随机数字表法分为试验组和对照组,每组各30例。对照组采用常规治疗,实施基础治疗加口服非甾体类消炎止痛药;试验组在对照组基础上采用刺血疗法联合四黄清热散外敷治疗,记录两组患者治疗前后中医症候评分、VAS评分、血尿酸、血常规(白细胞计数)、C-反应蛋白、红细胞沉降率及不良反应发生情况。结果两组治疗前相关观察指标比较,差异无统计学意义(P>0.05);试验组治疗后的中医症候评分、VAS评分、血尿酸、C-反应蛋白及红细胞沉降率均低于对照组,差异有统计学意义(P<0.05);两组患者白细胞计数、不良反应总发生率比较,差异无统计学意义(P>0.05)。结论刺血疗法联合四黄清热散外敷可有效缓解急性痛风性关节炎患者疼痛,改善患者临床症状,降低尿酸值,安全性较高。Objective To explore the clinical study of pricking blood therapy combined with Sihuang Qingre Powder in the treatment of acute gouty arthritis(AGA).Methods A total of 60 AGA patients who were hospitalized in the Second Department of Orthopedics,Zhanjiang No.1 Hospital of Traditional Chinese Medicine from August 2018 to May 2021 were selected and randomly divided into the experimental group and the control group,with 30 cases in each group.The control group was treated with conventional treatment:basic treatment plus oral nonsteroidal anti-inflammatory analgesics.The experimental group was treated with pricking blood therapy combined with Sihuang Qingre Powder for external application on the basis of the control group.The scores of TCM symptoms,VAS score,blood uric acid(UA),blood routine(white blood cell count),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and adverse reactions of two groups were recorded before and after treatment.Results There was no significant difference in the related observation indexes between the two groups before treatment(P>0.05).After treatment,TCM symptom score,VAS score,blood UA,CRP and ESR in the experimental group were lower than those in the control group,with statistically significant differences(P<0.05).There were no significant difference in white blood cell count and total incidence of adverse reactions between the two groups(P>0.05).Conclusion The pricking blood therapy combined with Sihuang Qingre Powder can effectively relieve the pain of patients with AGA,improve the clinical symptoms of patients,and reduce the UA level,with higher safety.
