摘  要:目的研制一种轻便、可计数、可调节负重力量,进行下肢主动功能锻炼的仪器并研究其在膝关节置换患者围手术期的应用效果。方法选取2019年6月至2020年6月在中山市中医院关节专科住院,符合纳入标准的100例膝关节置换患者,按入院的先后顺序随机分成常规组和锻炼仪组,常规组按膝关节置换术后常规康复训练,锻炼仪组在常规组基础上,增加下肢肌力康复锻炼仪的干预,比较两组患者术后首次下地时间、术后3 d,7 d、1个月膝关节HSS评分、术后14 d日常生活自理能力(ADL)量表、住院天数。结果两组患者术后首次下地时间比较,锻炼仪组(2.52±0.89)d,常规组(3.24±0.98)d,差异有统计学意义(P<0.05);两组患者术前膝关节HSS评分比较,差异无统计学意义(P>0.05);术后膝关节HSS评分比较:锻炼仪组术后3 d(60.54±5.02)分,7 d(66.78±4.89)分,1个月(71.88±4.88)分,常规组术后3 d(54.24±5.30)分,7 d(60.70±4.58)分,1个月(65.90±4.37)分,锻炼仪组均优于常规组(P<0.05);两组患者术后14 dADL量表比较:锻炼仪组(91.20±3.23)分,常规组(84.90±5.25)分,差异有统计学意义(P<0.05);两组患者住院天数比较:锻炼仪组(9.48±2.08)天,常规组(10.68±3.01)天,差异有统计学意义(P<0.05)。锻炼仪使用过程中,患者无不适感,操作简单,治疗安全,无任何不良反应。结论下肢肌力康复锻炼仪应用于膝关节置换患者,能缩短患者术后首次下地时间,促进术后膝关节功能的恢复,患者生活自理能力得到了提高,从而生活质量也进一步得到了改善。Objective To develop a lightweight apparatus with the function of counting and adjustable weightbearing force for active functional exercise of the lower limbs,and to study the effect of its application in the perioperative period of patients with knee joint replacement.Methods A total of 100 patients with knee joint replacement who were admitted in the Joint Specialty of Zhongshan Hospital of Traditional Chinese Medicine from June 2019 to June 2020 and met the inclusion criteria were randomly divided into the conventional group and the exercise apparatus group according to the time order of admission.The conventional group received the conventional rehabilitation training after knee joint replacement,while the exercise apparatus group received the intervention of the rehabilitation exercise apparatus for lower limb muscle strength on the basis of the training in the conventional group.The first ambulation time of the patients after surgery,the Hospital for Special Surgery(HSS)score of the knee joint at 3 days,7 days and 1 month after surgery,the activity of daily living(ADL)scale at 14 days after surgery and days of hospital stay were compared between the two groups.Results The first ambulation time of the patients after surgery was(2.52±0.89)days in the exercise apparatus group and(3.24±0.98)days in the conventional group,with statistically significant differences(P<0.05).There was no statistically significant difference between the two groups in the preoperative knee HSS scores(P>0.05).The postoperative knee HSS scores at 3 days,7 days and 1 month after surgery in the exercise apparatus group were(60.54±5.02),(66.78±4.89)and(71.88±4.88)respectively,while those in the conventional group were(54.24±5.30),(60.70±4.58)and(65.90±4.37)respectively,indicating that those in the exercise apparatus group were better than those in the conventional group(P<0.05).The ADL score of patients at 14 days after surgery was(91.20±3.23)in the exercise apparatus group and(84.90±5.25)in the conventional group,with statisti
