摘　　要：目的探讨颈动脉内膜剥脱术(CEA)开展初期的常见手术问题、相关应急处理方案及预后,为进一步优化手术治疗方案提供启示。方法选择2018年10月至2020年10月于南京医科大学第二附属医院40例行CEA的患者为研究对象,回顾性总结分析术前评估、麻醉、手术方式、应急处理以及术后处理等常见问题对预后的影响。结果40例患者中38例患者动脉狭窄获得有效缓解;3例患者出现相关并发症,经治疗后均缓解。结论CEA是治疗中、重度颈动脉狭窄的良好方法,手术难度较低,但风险性较高。尤其在手术开展初期,术前、术中、术后均有细节问题需引起高度重视。Objective To explore the common surgical issues,relevant emergency treatment and prognosis for patients treated with carotid endarterectomy(CEA)at the initial stage,and to offer enlightenment for further optimization of surgical treatment.Methods A total of 40 patients who underwent CEA in the Second Affiliated Hospital of Nanjing Medical University from October 2018 to October 2020 were selected as research subjects.The effects of common issues including preoperative evaluation,anesthesia,surgical mode,emergency treatment and postoperative management on prognosis were retrospectively summarized and analyzed.Results 38 in 40 patients with carotid artery stenosis(CAS)were effectively relieved.3 patients with relevant complications were relieved after treatment.Conclusion CEA is a favorable method for the treatment of moderate and severe CAS.The difficulty of the surgery is low but with a relatively high risk.The detail issues in the pre-operative,intra-operative and post-operative stages require great attention,especially at the initial stage of the surgery.
