哌拉西林钠他唑巴坦钠致骨关节痛1例  

One Case of Arthralgia Induced by Piperacillin Sodium and Tazobactam Sodium for Injection

摘　　要：1例78岁女性患者,主因反复咳嗽、咳痰50余年,加重1d,诊断为肺部感染入院,入院后根据痰培养结果给予注射用哌拉西林钠他唑巴坦钠抗感染治疗,该药治疗2d后,患者出现四肢及躯干疼痛,骨痛样症状,皮肤肌肉无触痛、无红肿。止痛治疗后,患者症状减轻,停药出院后疼痛症状消失。1个月后,患者再次因为肺部感染入院治疗,给予注射用哌拉西林钠他唑巴坦钠治疗,给药后患者再次出现上述疼痛症状,且较1月前加重,夜间不能入睡,药师会诊后考虑为该药引起的不良反应,停用该药后疼痛症状消失。Female patient,78 years dd,“Recurrent cough and sputum for more than 50 years,aggravated for 1 d”,was diagnosed as pulmonary infection.After admission,anti-infective treatment with injectable piperacillin sodium tazobactam sodium was given according to sputum culture results.After 2 d,the patient developed pain in the extremities and trunk with bone pain-like symptoms,no tenderness,no redness in the skin muscles.After analgesic treatment,the patient′s symptoms were relieved,and the pain disappeared after discharge.One month later,the patient was again admitted to the hospital for pulmonary infection and was treated with piperacillin sodium tazobactam sodium for injection.After the first dose,the patient again developed the above symptoms,and the pain symptoms were aggravated compared with that one month ago,and could not sleep at night.After consultation with the pharmacist,considering the related adverse reactions caused by the drug,advising to discontinue the drug,the pain symptoms disappeared after discontinuation of the drug.
