鼓膜置管联合腺样体切除治疗分泌性中耳炎患儿的疗效观察

摘　　要：目的分析小儿分泌性中耳炎的疾病特点,评价鼓膜置管联合腺样体切除治疗的优势。方法80例分泌性中耳炎患儿,随机分为对照组及观察组,各40例。对照组采取鼓膜置管术治疗,观察组采取鼓膜置管联合腺样体切除治疗。比较两组患儿的临床疗效、并发症发生情况、1年后复发情况、积液消失时间、耳鸣消失时间、黏膜愈合时间、恢复听力时间、纯音听力。结果观察组患儿的总有效率95.00%高于对照组的72.50%,差异具有统计学意义(P<0.05)。观察组患儿的并发症发生率5.00%、1年后复发率2.50%均低于对照组的22.50%、17.50%,差异具有统计学意义(P<0.05)。观察组患儿的积液消失时间、耳鸣消失时间、黏膜愈合时间、恢复听力时间分别为(7.30±1.30)、(9.50±1.50)、(12.30±1.50)、(24.50±2.50)d,均短于对照组的(13.50±1.50)、(16.80±1.80)、(20.50±2.20)、(33.50±3.20)d,纯音听力(21.70±3.30)dBHL高于对照组的(19.20±2.60)dBHL,差异具有统计学意义(P<0.05)。结论鼓膜置管联合腺样体切除治疗分泌性中耳炎患儿临床疗效理想,可促进患儿康复,安全性理想。Objective To analyze the characteristics of secretory otitis media in children,and evaluate the advantages of tympanic membrane insertion combined with adenoidectomy.Methods A total of 80 children with secretory otitis media were randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with tympanic membrane insertion,and the observation group was treated with tympanic membrane insertion and adenoidectomy.Both groups were compared in terms of clinical efficacy,occurrence of complications,recurrence 1 year after treatment,disappearance time of effusion,disappearance time of tinnitus,mucosal healing time,hearing recovery time,and pure tone hearing.Results The total effective rate 95.00% of the observation group was higher than 72.50% of the control group,and the difference was statistically significant(P<0.05).In the observation group,the complication rate was 5.00% and the recurrence rate was 2.50% 1 year after treatment,which were lower than 22.50% and 17.50% in the control group,and the differences were statistically significant(P<0.05).The disappearance time of effusion,disappearance time of tinnitus,mucosal healing time,and hearing recovery time in the observation group were(7.30±1.30),(9.50±1.50),(12.30±1.50),(24.50±2.50)d,which were shorter than(13.50±1.50),(16.80±1.80),(20.50±2.20),(33.50±3.20)d in the control group;the pure tone hearing(21.70±3.30)dBHL in the observation group was better than(19.20±2.60)dBHL in the control group;all the differences were statistically significant(P<0.05).Conclusion Tympanic membrane insertion combined with adenoidectomy has an ideal clinical effect in the treatment of children with secretory otitis media,and can promote the recovery of children with ideal safety.
