摘 要:目的:探讨左氧氟沙星联合头孢美唑治疗慢性支气管炎对患者FVC及FEV1水平的影响。方法:选取2020年2月—2021年2月本院收治的134例慢性支气管炎患者,采用随机数字表法将其分为对照组和研究组各67例。两组均实施平喘及祛痰等常规治疗,对照组静脉注射0.5 g的左氧氟沙星,1次/d,7 d为1疗程,连续治疗4个疗程;研究组在对照组的基础上静脉注射2 g头孢美唑,每12 h注射1次,7 d为1疗程,连续治疗4个疗程。观察分析两组肺功能[用力肺活量(FVC)、1秒用力呼气容积(FEV1)、FEV1/FVC及呼气流量峰值(PEF)]、炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)、白细胞介素-6(IL-6)及超敏C-反应蛋白(hs-CRP)]、症状持续时间(发热、咳嗽及气喘持续时间)、临床有效率及不良反应。结果:治疗前,两组FVC、FEV1、FEV1/FVC和PEF比较,差异无统计学意义(P>0.05);治疗后,两组FVC、FEV1、FEV1/FVC及PEF水平高于治疗前,差异有统计学意义(P<0.05);且研究组高于对照组,差异有统计学意义(P<0.05)。治疗前,两组IL-8、IL-6、Hs-CRP及TNF-α水平比较,差异无统计学意义(P>0.05);治疗后,两组IL-8、IL-6、Hs-CRP及TNF-α水平低于治疗前,差异有统计学意义(P<0.05);且研究组低于对照组,差异有统计学意义(P<0.05)。与对照组比较,研究组发热、咳嗽及气喘持续时间短(P<0.05)。与对照组比较,研究组临床有效率高(P<0.05),两组不良反应发生率无明显差异(P>0.05)。结论:慢性支气管炎患者应用左氧氟沙星与头孢美唑联合治疗效果显著,可有效改善其临床症状,提高肺功能,减轻机体的炎症反应,不良反应少,具有较高的安全性,值得应用。Objective:To investigate the effect of levofloxacin combined with cefmetazole on the levels of FVC and FEV1 in patients with chronic bronchitis.Methods:According to the random number table method,a total of 134 patients with chronic bronchitis admitted to our hospital from February 2020 to February 2021 were selected and randomly divided into the control group and the study group,with 67 cases in each group.Both groups were treated with routine treatment,such as relieving asthma and eliminating phlegm.The control group was intravenously injected levofloxacin(0.5 g/day,seven days as a treatment)for four consecutive treatment courses.Based on the control group,the study group was intravenously injected cefmetazole(2 g/time,once every 12 hours,seven days as a course of treatment)for four consecutive treatment courses.Pulmonary function[forced lung capacity(FVC),forced exhalatory volume in one second(FEV1),FEV1/FVC,and peak expiratory flow(PEF)],inflammatory factor[tumor necrosis factor-α(TNF-α),interleukin-8(IL-8),interleukin-6(IL-6)and hypersensitive C-responsive protein(hs-CRP)],duration of symptoms(duration of fever,cough,and asthma),clinical effective rate,and adverse reactions in both groups were observed and analyzed.Results:Before treatment,there was no significant difference in FVC,FEV1,FEV1/FVC,and PEF in both groups(P>0.05).After treatment,the levels of FVC,FEV1,FEV1/FVC,and PEF in both groups were significantly higher than those before treatment(P<0.05),and these levels in the study group were significantly higher than those in the control group(P<0.05).Before treatment,there was no significant difference in the levels of IL-8,IL-6,Hs-CPR,and TNF-α in both groups(P>0.05).After treatment,the levels of the above inflammatory factors in both groups were lower than those before treatment(P<0.05),and the inflammatory factor levels in the study groups were significantly lower than those in the control group(P<0.05).Compared with the control group,the duration of fever,cough,and asthma in the study group were
