超声引导在无或少量胸腔积液行人工气胸内科胸腔镜检查的疗效观察

摘　　要：目的探讨超声引导在无或少量胸腔积液行人工气胸内科胸腔镜检查中的效果。方法选取2020年4月~2021年6月本院收治的无或少量胸腔积液的胸膜疾病患者82例,按随机数字表法分为对照组和观察组各41例。对照组行传统人工气胸,观察组行超声引导联合中心静脉导管人工气胸。对比两组的血流动力学、并发症。结果术前,两组心率(HR)、呼吸频率(RR)、平均动脉压(MAP)相比,差异无统计学意义(P>0.05);术后,观察组的HR、RR、MAP均低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为7.32%,低于对照组的24.39%,差异有统计学意义(P<0.05)。结论超声引导联合中心静脉导管人工气胸在无或少量胸腔积液行人工气胸内科胸腔镜检查患者中安全有效,能够维持血流动力学稳定,降低并发症发生率,值得应用。Objective To investigate the efficacy of ultrasound guidance inartificial pneumothorax for thoracoscopy in patients without or with a small amount of pleural effusion.Methods 82 patients with pleural diseaseadmitted to our hospital from April 2020 to June 2021,without or with a small amount of pleural effusion,were randomly divided into control group and observation group.The 41 cases in control group were treated with traditional artificial pneumothorax,whereas the 41 cases in observation group were treated with ultrasound-guidedvenous catheter pneumothorax.Hemodynamic and complications of the two groups were compared.Results No significant difference was observed in heart rate(HR),respiratory frequency(RR)and mean arterial pressure(MAP)before surgery(P>0.05).HR,RR and MAP in the observation group are significantly lower than those in control group(P<0.05).The rate of complications was 7.32%in the observation group,which was significantly lower than the 24.39%of the control group(P<0.05).Conclusion Ultrasound-guided venous catheter pneumothorax is safe and effective in patients without or with a small amount of pleural effusion.The techniquecan maintain hemodynamic stability and reduce the incidence of complications,which is worthy of application.
