半夏泻心汤联合二甲双胍在磺脲类药物继发性失效中的临床观察

摘　　要：目的:观察半夏泻心汤对口服磺脲类继发性失效2型糖尿病的作用机制及临床应用特征,并从理论上探讨半夏泻心汤增加胰岛β细胞数目和改善胰岛β细胞功能有关。方法:将132例继发性失效的2型糖尿病患者随机分成半夏泻心汤组(A组):在原来足量磺脲类药物基础上加用半夏泻心汤;二甲双胍组(B组):在原来足量磺脲类药物基础上加二甲双胍每日750 mg;联用组(C组):在原来足量磺脲类药物基础上加半夏泻心汤和二甲双胍每日750 mg,分3次口服,采用对比研究的方法,观察三组患者治疗前后的疗效和相关指标的变化。结果:治疗前,三组间差异均无统计学意义。治疗前后对比,空腹血糖、餐后2 h血糖(2 hours Postprandial Blood Glucose,2 h PBG)、糖化血红蛋白、胰岛素(Fasting Serum Insulin,FINS)及胰岛素敏感指数(Insulin Sensitivity Index,ISI)均明显改善,差异有统计学意义(P≤0.05);治疗后组间比较,联合组与半夏泻心汤组的差异有统计学意义(P≤0.05);联合组与二甲双胍组的差异有统计学意义(P≤0.05);半夏泻心汤组与二甲双胍组的差异无统计学意义。结论:半夏泻心汤组、二甲双胍组、联合组均可改善血糖、糖化血红蛋白、胰岛素及胰岛素敏感指数,联合组优于其他两组。Objective:To observe the mechanism and clinical application of the Banxia Xiexin decoction(半夏泻心汤)in treating type 2 diabetes with secondary failure of sulfonylureas.It is also theoretically discussed that the Banxia Xiexin decoction is related to increasing the number and improving the function of islet beta cells.Methods:132 cases were randomly divided into the Banxia Xiexin decoction group(group A):On the basis of the original sufficient amount of SU,the Banxia Xiexin decoction was given;the metformin group(group B):based on the original sufficient amount of SU,metformin was added,750mg/d;the combination group(group C):the Banxia Xiexin decoction and metformin of 750mg/d were taken orally for three times on the basis of the original sufficient amount of SU,and the efficacy and related indexes in the three groups before and after treatment were observed by comparative study.Results:Before treatment,there was no significant difference between the three groups.After treatment,fasting blood glucose,2h PBG,glycated hemoglobin,insulin(INS)and insulin sensitivity index(ISI)were significantly improved,with statistical significance(P≤0.05);After treatment,the difference between the combination group and the Banxia Xiexin decoction group was statistically significant(P≤0.05).The difference between the combination group and the metformin group was statistically significant(P≤0.05).There was no significant difference between the Banxia Xiexin decoction group and the metformin group.Conclusion:The Banxia Xiexin decoction group,metformin group and combination group can all improve blood glucose,glycated hemoglobin,insulin and insulin sensitivity index,and the combination group is better than the other two groups.
