摘 要:目的观察3D打印技术辅助治疗陈旧髋臼骨盆骨折的临床疗效。方法2013年2月至2013年12月北京积水潭医院创伤骨科对20例陈旧髋臼骨盆患者进行了3D打印技术辅助的手术治疗。将所有患者骨盆打印为实体模型。使用多枚克氏针钻孔定位,进行截骨。然后进行模拟复位,塑形钢板进行固定。采用视觉疼痛模拟评分(VAS)对患者疼痛情况进行评定,采用Harris评分和EQ-5D评价患者髋部功能及生活质量。结果3D打印技术可以在术前对骨折截骨位置,复位方式,固定方式进行有效的模拟,显著缩短手术时间,减少手术创伤,减少手术出血;在手术时间手术出血较传统方式均有明显改善。VAS评分术前为(4.3±2.1)分;术后末次随访时降至(1.2±0.9)分(P<0.05)。Harris评分术前为(26.8±14.3)分;术后末次随访时为(77.2±11.3)分(P<0.05)。EQ-5D术前为(0.36±0.12)分;术后末次随访为(0.72±0.15)分(P<0.05)。8例优4例良;1例差(最终行全髋关节置换术);7例失随访。结论3D打印技术可以有效的辅助进行陈旧髋臼骨折手术治疗,对手术时间、手术损伤、手术效果均有一定改善。Objective To observe the clinical effect of 3D printing technology in the treatment of old acetabular and pelvic fractures.Methods From February 2013 to December 2013,20 patients with chronic acetabular and pelvic fracture were treated with 3D printing technology in the Department of trauma and orthopedics of Beijing Jishuitan Hospital.Print all pelvis of patients as a solid model.Multiple Kirschner wires were used for drilling and positioning for osteotomy,and had analog reset,bending plate for fixation then.Visual pain analogue scale(VAS)was used to evaluate the pain,Harris score and EQ-5D were used to evaluate the hip function and quality of life.Results 3D printing technology can effectively simulate the fracture osteotomy position,reduction mode and fixation mode before operation,significantly shorten the operation time,reduce the operation trauma and reduce the operation bleeding.The operation bleeding at the operation time is significantly improved compared with the traditional method.The VAS score was(4.3±2.1)before operation and(1.2±0.9)at the last follow-up after operation(P<0.05).Harris score was(26.8±14.3)before operation,(77.2±11.3)at the last follow-up after operation(P<0.05).EQ-5D was(0.36±0.12)before operation,and(0.72±0.15)at the last follow-up after operation(P<0.05).There were 8 excellent cases,4 good cases,1 poor case(total hip arthroplasty was finally performed),and 7 cases lost follow-up.Conclusion 3D printing technology can effectively assist the surgical treatment of old acetabular fractures,and improve the operation time,operation injury and operation effect.
