胃和Ⅲ号治疗慢性萎缩性胃炎伴肠上皮化生寒热错杂证的临床观察

摘　　要：目的:观察胃和Ⅲ号治疗慢性萎缩性胃炎(Chronic Atrophic Gastritis,CAG)伴肠上皮化生(Intestinal Metaplasia,IM)寒热错杂证的临床疗效。方法:采用随机、对照法将70例CAG伴IM寒热错杂证患者,分为观察组、对照组,每组35例。因两组在治疗过程中各有5例脱落,实际每组30例患者完成研究。观察组口服胃和Ⅲ号(半夏、干姜、黄芩、黄连、半枝莲、莪术等),每日3次,每次6 g;对照组口服胃复春片(香茶菜、炒枳壳、红参),每日3次,每次4片,疗程均为6个月。通过观察两组患者治疗前后的临床症状、胃镜、病理及幽门螺杆菌感染情况明确其临床疗效。结果:经过治疗后,两组患者均取得良好疗效,其中观察组总有效率为86.67%(26/30),对照组总有效率为76.67%(23/30)(P<0.05),两组差异有统计学意义。治疗后观察组与对照组比较,中医症状积分减少(P<0.05),观察组在改善胃脘痞满、遇冷加重、嗳气3种症状方面优于对照组(P<0.05),观察组慢性萎缩性胃炎患者治疗后的胃镜镜下表现积分、胃黏膜病理组织学积分均低于对照组,差异具有统计学意义(P<0.05),观察组能明显根除Hp感染及减轻肠上皮化生程度(P<0.05)。结论:胃和Ⅲ号治疗CAG伴IM寒热错杂证,效果显著,值得进一步研究和探索。Objective:To observe the clinical efficacy of Weihe III(胃和Ⅲ号)in treating chronic atrophic gastritis(CAG)with intestinal metaplasia(IM)with mixed cold and heat syndrome.Methods:70 cases of CAG with IM with mixed cold and heat syndrome were randomly divided into the observation group and the control group,with 35 cases in each group.Because there were 5 cases of falling off in each group,30 patients in each group completed the study.In the observation group,Weihe III(Pinellia ternata,Zingiberis Rhizoma,Scutellariae Radix,Coptidis Rhizoma,Scutellariae Barbatae,Curcuma Rhizome,etc.)was taken orally three times a day,6 g each time;In the control group,Weifuchun tablets(胃复春片)(fragrant teas,fried bitter orange and red ginseng)were taken orally,3 times a day,4 tablets each time,and the course of treatment was 6 months.By observing the clinical symptoms,gastroscopy,pathology and Helicobacter pylori(Hp)infection before and after treatment,the clinical efficacy of the two groups was confirmed.Results:After treatment,both groups achieved good curative effect,the total effective rate of the observation group was 86.67%(26/30),and that of the control group was 76.67%(23/30)(P<0.05),and the difference between the two groups was statistically significant.After treatment,compared with that of the control group,the symptom score of TCM in the observation group decreased(P<0.05),Weihe III is superior in relieving the symptoms of fullness in stomach,aggravation in cold and belching(P<0.05),while the scores of gastroscopy and gastric mucosa histopathology of patients with chronic atrophic gastritis in the observation group after treatment were lower than those in the control group,which was with statistical difference(P<0.05),which can obviously eradicate Hp infection and reduce intestinal metaplasia(P<0.05).Conclusion:Weihe III has a remarkable effect in treating CAG with IM with mixed cold and heat syndrome,which deserves further study and exploration.
