摘　　要：目的探讨不同刮痧方法干预腰椎间盘突出症(lumbar disc herniation,LDH)的效果。方法2017年7月至2018年6月,便利抽样法选取扬州市某医院骨伤科门诊就诊的非急症期LDH患者90例,随机分为循经刮痧组、穴位刮痧组及循经加穴位刮痧组各30例。刮拭三个疗程后,比较三组日本骨科学会下腰痛评估表评分(Japanese orthopaedic association,JOA)、视觉模拟疼痛评分(visual analogue scale,VAS)及血清白介素1-beta(interleukin 1-beta,IL-1β)、血清白介素10(interleukin 10,IL-10)含量。结果与干预前相比,干预后三组患者的JOA评分以及血清IL-10含量均上升,VAS评分以及血清IL-1β含量均下降,差异均有统计学意义(均P<0.01)。干预后,循经加穴位刮痧组JOA评分、VAS评分以及血清IL-1β、IL-10含量的改善情况均优于循经刮痧组和穴位刮痧组,差异均有统计学意义(均P<0.01);循经刮痧组和穴位刮痧组JOA评分、VAS评分及血清IL-1β、IL-10含量比较,差异均无统计学意义(均P>0.05)。结论刮痧干预LDH疗效确切,具有抗炎、镇痛作用,经穴结合刮痧干预LDH比单纯循经或穴位刮痧效果好,该方法虽然刮拭部位较多,但疗效更优,值得推广。Objective To explore the effects of different scraping methods on lumbar disc herniation(LDH).Methods Convenient sampling method was used to select 90 patients with non-acute-phase LDH who visited the Traumatic Orthopedic Clinic of a hospital in Yangzhou from July,2017 to June,2018.They were randomly divided into a channel scraping group,an acupoint scraping group and a channel with acupoint scraping group,with 30 patients in each group.After 3 courses of scraping,the Japanese Orthopaedic Association(JOA)score,Visual Analogue Scale(VAS)score,serum interleukin 1-beta(IL-1β)and interleukin 10(IL-10)levels were compared among the three groups.Results The JOA score and the serum IL-10 level of three groups significantly increased,while the VAS score and the serum IL-1βlevel significantly decreased after intervention(all P<0.01).The changes of the JOA score,VAS score and serum IL-1βand IL-10 levels in the channel with acupoint scraping group were more evident than those in the channel scraping group and the acupoint scraping group(all P<0.01).There were no significant differences in the JOA score,VAS score and serum IL-1βand IL-10 levels between the channel scraping group and the acupoint scraping group(all P>0.05).Conclusions The therapeutic effects of scraping for LDH is definite,including anti-inflammation and analgesia.The channel with acupoint scraping reports better effects on LDH than simple channel or acupoint scraping.Although this method has to scrape more areas,it has better effects and is worthy of popularizing.
