社区留守老年糖尿病患者对居家医养护需求及影响因素分析

摘　　要：目的了解山东省某市社区留守老年糖尿病患者对居家医养护需求现状,并分析其影响因素。方法采用整群抽样的方法于2020年1月—2021年4月在山东省某市东西南北中5个方位不同的26个社区选取538例留守老年糖尿病患者为研究对象,分析其对居家医养护的需求及影响因素。结果社区留守老年糖尿病患者对专业护理服务和精神抚慰服务的需求相对较大,其次是康复理疗服务、基础护理服务和生活照料服务。单因素分析显示,年龄、学历、留守时间、日常生活活动能力、是否了解糖尿病相关知识是社区留守老年糖尿病患者对居家医养护的需求水平的影响因素(P<0.05)。多元回归分析显示,学历、留守时间、了解糖尿病相关知识与社区留守老年糖尿病患者对居家医养护需求正相关(P<0.05),日常生活活动能力与与社区留守老年糖尿病患者对居家医养护需求负相关(P<0.05)。结论社区留守老年糖尿病患者对居家医养护需求较大,在为这类患者进行居家医疗服务时,应该重点关注学历层次低、留守时间长、日常生活活动能力差、不了解糖尿病相关知识的人群,为这类人群提供更加完善的照护体系。Objective To understand the status quo of the demand for home medical care of left-behind elderly diabetic patients in a community of a city in Shandong Province,and analyze its influencing factors.Methods The cluster sampling method was used to select 538 left-behind elderly diabetic patients in 26 communities with different orientations from January 2020 to April 2021 in a certain city of Shandong Province in 26 communities with different orientations,analyed of its demand for home medical care and its influencing factors.Results Left-behind elderly diabetic patients in the community had relatively greater demand for professional nursing services and spiritual comfort services,followed by rehabilitation physiotherapy services,basic nursing services and life care services.Univariate analysis showed that age,educational background,staying time,activities of daily living,and understanding of diabetes-related knowledge were the influencing factors of the level of demand for home medical care of left-behind elderly diabetic patients in the community(P<0.05).Multiple regression analysis showed that educational background,stay-at-home time,and knowledge of diabetes were positively correlated with the needs of home-based medical care and nursing among left-behind elderly diabetic patients in the community(P<0.05).Maintenance requirements were negatively correlated(P<0.05).Conclusion The elderly left-behind diabetic patients in the community have a greater need for home medical care.When providing home medical services for such patients,we should focus on people with low educational level,long staying time,poor activities of daily living,and ignorance of diabetes-related knowledge,which provides a more complete care system for this group of people.
