摘 要:目的探讨对2型糖尿病合并高血压患者由临床药师参与治疗管理对血压、血糖控制的影响。方法选择该院2020年1—12月诊治2型糖尿病合并高血压者90例,按照随机数表法分为常规管理对照组(n=45)与临床药师参与治疗管理观察组(n=45),在患者治疗12个月后,调查两组药物调整情况,评估患者干预前后的血糖指标(空腹血糖、餐后2 h血糖、糖化血红蛋白)以及血压指标(收缩压、舒张压)水平。结果观察组药物调整、增加剂量、减小剂量、更换药物发生率分别为75.56%、31.11%、22.22%、17.78%,低于对照组46.67%、11.11%、6.67%、2.22%,差异有统计学意义(χ^(2)=9.833、5.591、4.406、4.444,P<0.05)。干预后观察组空腹血糖为(6.50±1.31)mmol/L、餐后2 h血糖(8.58±1.45)mmol/L、糖化血红蛋白(6.20±1.02)%,较对照组更低;收缩压为(127.13±7.72)mmHg、舒张压为(76.33±7.80)mmHg,较对照组低,差异有统计学意义(t=4.275、5.483、4.128、3.475、3.475,P<0.05)。结论对2型糖尿病合并高血压者由临床药师参与治疗管理能够及时进行用药方案的调整,可提高血压、血糖的控制效果,值得推广。Objective To investigate the influence of clinical pharmacists on blood pressure and blood glucose control in patients with type 2 diabetes and hypertension.Methods From January to December 2020,90 patients with type 2 diabetes and hypertension were selected from the hospital,and they were divided into a routine management control group(n=45)and a clinical pharmacist participating in treatment management observation group(n=45)according to the random number table method.After the patient was treated for 12 months,the adjustment of the two groups of drugs was investigated,and the blood glucose indicators(fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin)and blood pressure indicators(systolic blood pressure,diastolic blood pressure)levels before and after the intervention were evaluated.Results The incidence of drug adjustment,dose increase,dose reduction,and drug replacement in the observation group were 75.56%,31.11%,22.22%,and 17.78%,respectively,which were lower than those of the control group by 46.67%,11.11%,6.67%,and 2.22%,and the difference was statistically significant(χ^(2)=9.833,5.591,4.406,4.444,P<0.05).After the intervention,the fasting blood glucose of the observation group was(6.50±1.31)mmol/L,the 2 h postprandial blood glucose(8.58±1.45)mmol/L,and glycosylated hemoglobin(6.20±1.02)%were lower than those of the control group.The systolic blood pressure was(127.13±7.72)mmHg and the diastolic blood pressure was(76.33±7.80)mmHg,which were lower than those of the control group,and the difference was statistically significant(t=4.275,5.483,4.128,3.475,3.475,P<0.05).Conclusion For patients with type 2 diabetes and hypertension,the participation of clinical pharmacists in the treatment and management can timely adjust the medication regimen,which can improve the effect of blood pressure and blood glucose control,and it is worthy of promotion.
