游离股前外侧皮瓣与腓肠神经营养血管皮瓣在足踝部创面修复中的应用比较

摘　　要：目的探讨游离股前外侧皮瓣与腓肠神经营养血管皮瓣在足踝部创面修复中的应用比较。方法选择2018年4月至2020年11月宜兴市人民医院收治的足踝部创面损伤患者18例进行研究,以随机数表法分为试验组(n=10)和对照组(n=8)。试验组给予游离股前外侧皮瓣治疗,对照组给予腓肠神经营养血管皮瓣治疗。比较两组患者的临床疗效、创面愈合时间、创面愈合率、感觉恢复情况、换药次数、住院时间、住院费用、换药疼痛和瘢痕评分及色素沉着发生情况。结果治疗后,试验组和对照组的愈合总有效率分别为90.00%、87.50%,差异无统计学意义(P>0.05);试验组创面愈合时间显著短于对照组,创面愈合率高于对照组,差异有统计学意义(P<0.05);两组感觉恢复比较差异无统计学意义(P>0.05);两组住院费用比较差异无统计学意义(P>0.05);试验组换药次数、住院时间显著低于对照组,差异有统计学意义(P<0.05);治疗后,试验组换药疼痛评分、瘢痕评分及色素沉着率均显著低于对照组,差异有统计学意义(P<0.05)。结论在足踝部创面修复中应用游离股前外侧皮瓣和腓肠神经营养血管皮瓣修复均有较好的效果,但游离股前外侧皮瓣愈合质量优于腓肠神经营养血管皮瓣。Objective To study comparison of free anterolateral femoral flap and sural nerve nutrient vessel flap in repairing ankle wounds.Methods 18 patients with foot wounds treated in Yixing People’s Hospital from April 2018 to November 2020 were selected for study.They were randomly divided into experimental group(n=10)and control group(n=8).The experimental group was treated with free anterolateral femoral flap,and the control group was treated with sural nerve nutrient vessel flap.The clinical efficacy,wound healing time,wound healing rate,sensory recovery,dressing change times,hospitalization time,hospitalization expenses,dressing change pain,scar score and pigmentation were compared between the two groups.Results After treatment,the total healing effective rate of experimental group and control group was 90.00% and 87.50%,respectively,there was no statistical significance(P>0.05).After treatment,the wound healing time of experimental group was significantly lower than that of control group,and the wound healing rate was significantly higher than that of control group(P<0.05).After treatment,there was no significant difference in sensory recovery between the two groups(P>0.05).After treatment,there was no significant difference in hospitalization cost between the two groups(P>0.05).The times of dressing change and hospital stay in experimental group were significantly lower than those in control group(P<0.05).After treatment,pain score,scar score and pigmentation rate of dressing change in experimental group were significantly lower than those in control group(P<0.05).Conclusion Both free anterolateral femoral flap and sural nerve vegetative vascular flap have good results in foot and ankle wound repair,but the healing quality of free anterolateral femoral flap is better than sural nerve vegetative vascular flap.
