90例氯氮平血药浓度监测结果回顾性分析

摘　　要：目的分析精神分裂患者氯氮平血药浓度监测情况,为临床精神分裂症治疗提供参考。方法采用回顾性分析法,收集宁夏回族自治区宁安医院2019年7月至2020年9月服用氯氮平并进行血药浓度监测的90例精神分裂患者的临床资料,对患者氯氮平及其代谢产物去甲氯氮平的血药浓度进行分析。结果90例精神分裂患者的氯氮平平均血药浓度为(353.53±305.89)ng·mL^(-1),仅有18.89%的患者血药浓度在治疗窗内。不同年龄、性别和就诊类别患者的氯氮平及去甲氯氮平血药浓度差异均无统计学意义(P均>0.05);男、女性患者的氯氮平与去甲氯氮平血药浓度比值差异均无统计学意义(P均>0.05)。结论氯氮平血药浓度个体差异较大,应加强其监测工作,选择适宜的给药剂量,制订个体化给药方案,在保证疗效的同时减少不良反应的发生。Objective To investigate the plasma concentration of clozapine in patients and provide reference for clinical treatment of schizophrenia.Methods Retrospective analysis method was used to collect the clinical data of clozapine taken in Ningxia Ningan Hospital and Ningxia Mental Health Center from July 2019 to September 2020,and the results were statistically analyzed.Results The average plasma concentration of clozapine was(353.53±305.89)ng·mL^(-1),and only 18.89%of the patients'blood concentration was in the treatment window;there was no significant difference in patient age,gender and type of treatment(P all>0.05).The plasma concentration ratio of clozapine and norclozapine had no significant difference in gender(P all>0.05).Conclusion The individualized blood concentration of clozapine varies greatly.It is necessary to strengthen the monitoring of blood concentration of clozapine.The appropriate dosage should be selected,and the individualized dosage regimen should be formulated to reduce the occurrence of adverse reactions while ensuring the efficacy.
