摘　　要：目的观察自拟益肾利石汤联合输尿管软镜碎石术(RIRS)治疗肾结石的临床效果。方法选取2017年11月至2019年10月上海市浦东新区公利医院收治的180例行RIRS的肾结石患者作为研究对象,按治疗方式分为观察组、对照组,每组90例。观察组患者接受RIRS+益肾利石汤治疗,对照组患者接受RIRS+术后常规处理治疗,比较两组患者术前7 d、术后24 h早期肾功能指标[尿液中性粒细胞明胶酶相关载脂蛋白(uNGAL)和尿液半胱氨酸蛋白酶抑制剂(uCys-C)];术后1个月评估对比两组患者的临床疗效、结石残留率和并发症发生率。结果术前7 d两组uNGAL、uCys-C差异均无统计学意义(P>0.05);术后24 h两组uNGAL、uCys-C均较术前升高(P<0.05),观察组uNGAL、uCys-C低于对照组(P<0.05)。术后1个月,观察组和对照组总有效率差异无统计学意义(P>0.05),而观察组治愈率高于对照组(P<0.05);术后1个月,观察组结石残留率、并发症发生率均低于对照组(P均<0.05)。结论自拟益肾利石汤能提高RIRS治疗肾结石的临床疗效,可减轻肾损伤、降低结石残留率和并发症的发生率。Objective To observe the efficacy of self-made Yishenlishi decoction plus retrograde intrarenal surgery(RIRS)for renal calculi.Methods In total,180 patients with renal calculi in Shanghai Pudong New Area Gongli Hospital between November 2017 and October 2019 received RIRS and were assigned by different treatments to observation group and control group,with 90 cases in each group.The observation group received“RIRS combining Yishenlishi decoction”treatment,and the control group received“RIRS combining routine postoperative management”treatment.Early renal function indexes(uNGAL and uCys-C)were observed at 7 days before operation and 24 hours after operation.The clinical efficacy,residual stone rate,and complication rate of the two groups were evaluated at 1 month after operation.Results There was no statistical difference in uNGAL and uCys-C between the two groups on the 7 days before operation(P>0.05).One day after operation,the uNGAL and uCys-C were higher than those before operation(P<0.05),and the uNGAL and uCys-C of the observation group were significantly improved compared with the control group(P<0.05).At 1 month after operation,there was no statistical difference in the total effective rates(P>0.05),but the cure rate in the observation group was significantly higher than that in the control group(P<0.05).One month after operation,the residual stone rate and the incidence of complications in the observation group were lower than those in the control group(P<0.05).Conclusion Self-made Yishenlishi decoction can improve the clinical efficacy of RIRS in the treatment of kidney stones.It can alleviate kidney damage,reduce the residual rate of stones and the incidence of complications.
