摘　　要：目的:探讨肝脏体积、脾肾静脉分流(splenorenal shunt,SRS)直径和SRS/PV值与肝硬化患者Child-Pugh分级的关系,旨在为临床改善肝硬化患者的生活质量提供依据。方法:选择2018年6月至2020年10月无锡市第五人民医院收治的152例肝硬化合并SRS患者,均行64层螺旋CT扫描,比较不同Child-Pugh分级患者的肝脏体积、SRS直径和SRS/PV值的差异,并应用Spearman法分析上述各指标与Child-Pugh分级的关系。结果:Child-Pugh分级与肝脏体积、SRS直径和SRS/PV值均有一定的相关性,其中与肝脏体积呈负相关(r=-0.813,P<0.001)、与SRS直径和SRS/PV值呈正相关(r=0.856、0.782,P<0.001)。结论:CT检测肝脏体积、SRS直径和SRS/PV值与肝硬化患者Child-Pugh分级存在相关性,临床可根据CT影像学具体参数评估肝硬化患者的病情和预后。Objective To study the relationship between the CT measurements of liver volume,splenorenal shunt(SRS)diameter and SRS/portal vein(PV)diameter ratio and Child-Pugh classification in patients with cirrhosis so as to improve their living quality.Methods One hundred and fifty-two patients with liver cirrhosis combined with SRS admitted to Wuxi Fifth People's Hospital from June 2018 to October 2020 were selected,all of whom underwent 64-layer spiral CT scans to compare the differences in liver volume,SRS diameter and SRS/PV diameter ratio in patients with different Child-Pugh classifications,and Spearman method was applied to analyzing the relationship between each of the above indicators and Child-Pugh classification.Results Child-Pugh classification correlated with liver volume(r=-0.813,P<0.001)negatively while with SRS diameter and SRS/PV diameter ratio(r=0.856,0.782,P<0.001)positively.Conclusion CT measurements of liver volume,SRS diameter and SRS/PV diameter ratio correlate with Child-Pugh classification in patients with cirrhosis and allow clinical assessment of the conditions and prognoses of patients with cirrhosis based on specific parameters of CT imaging.
