不同留针时间治疗风寒湿型颈椎病临床观察

摘　　要：目的:观察不同留针时间治疗风寒湿型颈椎病的临床疗效。方法:将90例风寒湿型颈椎病患者,随机分为观察1组、观察2组、观察3组,每组30例,观察1组针刺后留针时间60 min,观察2组针刺后留针时间45 min,观察3组针刺后留针时间30 min。分别观察3组治疗后VAS评分、田中靖久颈椎病量化表评分、颈部功能障碍指数(neck disabilitv index,NDI)变化情况,并比较3组临床疗效。结果:观察1组有效率为96.7%,观察2组有效率为93.3%,观察3组有效率为80.0%,观察1组、观察2组有效率均高于观察3组,差异有统计学意义(P<0.05)。观察1组、观察2组治疗后VAS评分、NDI评分低于观察3组,田中靖久颈椎病量化表评分高于观察3组,差异有统计学意义(P<0.05)。结论:针刺治疗风寒湿型颈椎病,留针时间60 min、45 min和30 min经过2个疗程治疗后,均能起到较好的临床疗效,但留针时间60 min、45 min临床疗效基本相同,并且优于留针时间30 min。临证时,留针时间以45 min为佳。Objective:To observe the clinical curative effect of different needle retention time on cervical spondylosis of wind-cold-dampness type.Methods:A total of 90 patients with cervical spondylosis of wind-cold-dampness type were randomly divided into the observation group 1,the observation group 2 and the observation group 3,with 30 cases in each group.The needle retention time after acupuncture in the observation group 1 was 60 minutes,that in the observation group 2 was 45 minutes and that in the observation group 3 was 30 minutes.The changes of visual analogue score(VAS) score,Tanaka Jingjiu cervical spondylosis quantitative scale score and neck disability index(NDI) in the three groups were observed respectively,and the clinical effects of the three groups were compared.Results:The effective rate of the observation group 1 was 96.7%,that of the observation group 2 was 93.3%,and that of the observation group 3 was 80.0%.The effective rates of the observation group 1 and the observation group 2 were higher than that of the observation group 3,and the difference was statistically significant(P<0.05).The VAS score and NDI score of the observation group 1 and the observation group 2 after treatment were lower than those of the observation group 3,and the scores of Tanaka Jingjiu cervical spondylosis quantitative table were higher than those of the observation group 3,and all the differences were statistically significant(P<0.05).Conclusion:Acupuncture for the treatment of cervical spondylosis of wind-cold-dampness type can have good clinical efficacy after two courses of treatment,but the clinical efficacy of 60 minutes and 45 minutes is basically the same and better than 30 minutes.In clinical practice,the needle retention time should be 45 minutes.
