“两针一灸”整合疗法治疗老年腰椎间盘突出症临床观察

摘　　要：目的:观察"两针一灸"整合疗法治疗老年腰椎间盘突出症(lumbar disc herniation,LDH)的临床疗效。方法:选取2020年1月至2020年12月河南中医药大学第一附属医院康复门诊和龙子湖院区康复门诊及病房诊治的老年LDH患者60例,采用随机数字表法分为对照组和治疗组,每组30例。对照组给予推拿治疗、腰椎牵引、中药热敷、物理疗法等治疗,治疗组在对照组的基础上给予"两针一灸"整合疗法,包括通督补肾针法、皮内针法、艾灸疗法。观察两组患者视觉模拟评分(visual analogue score,VAS)、Oswestry残疾指数(Oswestry disability index,ODI)、日本骨科协会评估治疗(Japanese Orthopaedic Association,JOA)分数进行评价,以患侧多裂肌表面肌电信号改变为客观观察指标,观察"两针一灸"整合疗法的有效性。结果:对照组有效率为86.7%,治疗组有效率93.3%,治疗组有效率高于对照组,差异有统计学意义(P<0.05);治疗组治疗后VAS评分、ODI评分均显著低于对照组,JOA评分高于对照组,差异有统计学意义(P<0.05)。结论:"两针一灸"整合疗法治疗老年腰椎间盘突出症疗效显著,可明显减轻患者腰腿疼痛程度,改善腰椎功能,提高生活质量。Objective:To observe the clinical curative effect of "two-needling and one-moxibustion" integrated therapy on senile lumbar disc herniation(LDH).Methods:A total of 60 elderly patients with LDH diagnosed and treated in the rehabilitation clinic of The First Affiliated Hospital of Henan University of CM and the rehabilitation clinic and ward of Longzihu Hospital from January 2020 to December 2020 were randomly divided into the control group and the treatment group,with 30 cases in each group.The control group was treated with massage,lumbar traction,Chinese medicinals hot compress and physical therapy,while the treatment group was additionally treated with "two-needling and one-moxibustion" integrated therapy on the basis of the control group,including needling technique for dredging GV and nourishing the kidney,intradermal needle therapy,and moxibustion therapy.The score of visual analogue score(VAS),Oswestry disability index(ODI) and Japanese Orthopaedic Association(JOA) score were observed and evaluated in the two groups.The EMG changes on the surface of multifidus muscle on the affected side were taken as objective observation indexes to observe the effectiveness of "two-needling and one-moxibustion" integrated therapy.Results:The effective rate of the control group was 86.7%,and that of the treatment group was 93.3%.The effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the VAS score and ODI score of the treatment group were significantly lower than those of the control group,and the JOA score was higher than that of the control group,and all the differences were statistically significant(P<0.05).Conclusion:"Two-needling and one-moxibustion" integrated therapy is remarkably effective in the treatment of senile LDH.It can significantly reduce the pain of patients′ waist and legs,improve lumbar function and the quality of life.
