浮针联合短刺夹脊穴治疗腰椎间盘突出症的临床研究

摘　　要：目的:观察短刺夹脊穴配合浮针治疗腰椎间盘突出症的临床疗效。方法:将120例患者随机分为传统针刺组60例、联合针刺组60例。传统针刺组予以传统针刺干预;联合针刺组予以短刺夹脊穴配合浮针干预。两组均每日治疗1次,每周4 d,疗程为4周,2个月后进行随访。分别于治疗前后观察两组视觉模拟疼痛量表(VAS)、腰椎日本骨科协会评分表(JOA)和Oswestry功能障碍指数问卷表(ODI),对比临床疗效差异,并对2个月后随访症状对比分析。结果:治疗后两组患者VAS评分和ODI均较治疗前明显降低,差异具有统计学意义(P<0.01),且联合针刺组优于传统针刺组,差异具有统计学意义(P<0.05)。治疗后两组患者JOA评分较治疗前显著增高,差异具有统计学意义(P<0.01),联合针刺组与传统针刺组比较,差异无统计学意义(P>0.05)。疗效评定方面,传统针刺组和联合针刺组总有效率分别为68.4%(39/57)和98.3%(57/58),差异有统计学意义(P<0.01)。结论:短刺夹脊穴配合浮针治疗腰椎间盘突出症可有效缓解腰椎疼痛程度,临床疗效明显优于传统针刺治疗。Objective:To observe the clinical effect of short needling Jiaji points combined with floating needling in the treatment of lumbar disc herniation(LDH).Methods:120 cases were randomly divided into the conventional acupuncture group and the combined acupuncture group,with 60 cases in each group.The conventional acupuncture group was treated with traditional acupuncture;the combined acupuncture group was treated with short needling Jiaji points and floating needling.Both groups were treated once a day,four days a week,for four weeks.The follow-up was carried out two months after the treatment.The scores of VAS,JOA and ODI were compared between before and after the treatment in the two groups.The clinical efficacy and the symptoms during the follow-up were compared between the two groups.Results:The scores of VAS and ODI were significantly lower after the treatment compared to those before the treatment in the two groups(P<0.01);of which the combined acupuncture group was better than the conventional acupuncture group(P<0.05).The JOA score was significantly increased after the treatment compared to that before the treatment in the two groups(P<0.01);of which there was no statistical difference between the two groups(P>0.05).The total effective rate was 98.3%(57/58)in the combined acupuncture group,which was significantly higher than 68.4%(39/57)in the control group(P<0.01).Conclusion:Short needling Jiaji points combined with floating needling can effectively relieve the pain of lumbar vertebrae in the treatment of LDH,which clinical effect is obviously superior to that of conventional acupuncture.
