摘 要:目的探讨2型糖尿病合并骨质疏松患者骨形成指标Ⅰ型胶原氨基端肽原(PINP)与骨吸收指标β-异构C-端肽(β-CTX)的表达以及意义。方法回顾性分析2019年8月—2021年8月该院收治的90例2型糖尿病患者作为研究对象,根据双能X线检测骨密度的结果将其分为骨质疏松组(30例)、骨量减少组(30例)、骨量正常组(30例)。记录3组性别、年龄、体质指数、糖尿病病程等一般资料,比较3组空腹血糖、糖化血红蛋白、胆固醇、三酰甘油、血磷、血钙、甲状旁腺素、碱性磷酸酶、股骨颈骨密度等生化指标和PINP、β-CTX水平,Pearson相关分析骨密度与一般资料、生化指标以及骨代谢指标的相关性。结果3组患者性别、年龄比较,差异有统计学意义(P<0.05);3组体质指数、糖尿病病程比较,差异有统计学意义(P<0.05);3组空腹血糖水平、糖化血红蛋白含量、股骨颈骨密度比较,差异有统计学意义(P<0.05),3组胆固醇、三酰甘油、甲状旁腺素、血磷、血钙、碱性磷酸酶比较差异无统计学意义(P>0.05);3组PINP、β-CTX水平比较,差异有统计学意义(P<0.05);股骨颈骨密度与年龄、病程、空腹血糖、糖化血红蛋白、PINP、β-CTX呈负相关(P<0.05),与体质量无明显相关性(P>0.05)。结论2型糖尿病合并骨质疏松患者PINP、β-CTX水平较高,股骨颈骨密度低,体质指数低且糖尿病病程长,PINP、β-CTX是影响股骨颈骨密度的因素。Objective To investigate the expression and significance of bone formation index type Ⅰ collagen proamino-terminal peptide(PINP) and bone resorption index β-isomeric C-terminal peptide(β-CTX) in patients with type 2 diabetes and osteoporosis. Methods A retrospective analysis of 90 patients with type 2 diabetes admitted to the hospital from August 2019 to August 2021 was used as the research object. According to the results of dual-energy x-ray detection of bone density, they were divided into osteoporosis group(30 cases), osteopenia group(30 cases), and normal bone mass group(30 cases). Recorded three groups of general information such as gender, age,body mass index, and diabetes course. Compared the three groups of fasting blood glucose, glycosylated hemoglobin,cholesterol, triglycerides, blood phosphorus, blood calcium, parathyroid hormone, alkaline phosphatase, femoral neck bone density and other biochemical indicators and PINP, β-CTX levels, Pearson correlation analysis of correlation between bone density and general data, biochemical indicators and bone metabolism indicators. Results There was a statistically significant difference in gender and age of the three groups of patients(P <0.05). Compared the body mass index and the course of diabetes among the three groups, the difference was statistically significant(P<0.05).The three groups of fasting blood glucose levels, glycosylated hemoglobin content, femoral neck bone mineral density, the difference was statistically significant(P <0.05), there was no statistically significant difference in cholesterol, triacylglycerol, parathyroid hormone, blood phosphorus, blood calcium and alkaline phosphatase among the three groups(P>0.05). Compared of the levels of PINP and β-CTX among the three groups, the difference was statistically significant(P <0.05). Femoral neck bone mineral density was negatively correlated with age, course of disease, fasting blood glucose, glycosylated hemoglobin, PINP, β-CTX(P<0.05), and no significant correlation with body weight(P
