同种异体腓骨段在重建骨质疏松肱骨近端骨折内侧柱中的应用

摘　　要：目的探讨切开复位应用同种异体骨腓骨段重建内侧柱后复位内固定治疗老年骨质疏松肱骨近端骨折的方法及临床疗效。方法选取2019年1月—2021年5月采用全麻,沙滩椅位下,常规三角肌胸大肌入路,切开应用同种异体骨腓骨段重建内侧柱后复位内固定治疗合并骨质疏松的肱骨近端Neer三部分、四部分骨折患者32例,对功能恢复进行评分。结果根据Constant-Murley的肩关节评分系统,对术后疗效进行评价,32例中优24例,良6例,优良率为93.75%。结论切开复位应用同种异体骨腓骨段重建内侧柱后复位内固定治疗老年骨质疏松的肱骨近端骨折,固定可靠,疗效满意。Objective To investigate the method and clinical effect of open reduction and internal fixation after reconstruction of medial column with allogeneic fibula segment in the treatment of proximal humeral fracture in elderly osteoporosis.Methods Selected from January 2019 to May 2021,general anesthesia was used,under the beach chair position,conventional deltoid pectoralis major approach was used.32 cases of neer three part and four part fractures of proximal humerus complicated with osteoporosis were treated by open reduction and internal fixation after reconstruction of medial column with allogeneic fibular segment.The functional recovery was scored.Results According to the shoulder scoring system of constant Murley,the postoperative curative effect was evaluated.Among the 32 cases,24 cases were excellent and 6 cases were good.The excellent and good rate was 93.75%.Conclusion open reduction,reconstruction of medial column with allogeneic fibula segment and reduction and internal fixation are reliable and effective in the treatment of proximal humeral fractures with osteoporosis in the elderly.
