基于计划行为理论的杭州市三级医院医务人员循证医学实践行为的影响因素分析

摘　　要：目的:基于计划行为理论调查杭州市三级医院医务人员循证医学实践行为的影响因素,为提高医务人员循证医学实践水平提供参考。方法:采用循证实践量表对杭州市260名三级医院医务人员进行调查,利用结构方程模型分析态度、主观规范、知觉行为控制、行为意向对循证医学实践行为的直接与间接影响,并检验各影响因素间的关系。结果:不同学历、职称、工作年限、实践意愿和培训参加状况的医务人员在循证医学实践行为上有显著差异(P<0.05);结构方程模型拟合较好,行为意向对循证医学实践行为有直接正向影响(β=0.583,P<0.001),态度、主观规范、知觉行为控制通过行为意向间接影响医务人员循证医学实践行为(β=0.122,P<0.001;β=0.153,P<0.001;β=0.384,P<0.001)。结论:当前医务人员循证医学实践水平有待提高,应从提高医务人员个人能力、认知和实践水平、加强对循证医学实践的社会支持等方面来改善医务人员的实践行为。Objective:Based on the theory of planned behavior,the influencing factors of evidence-based medicine practice of medical staff in tertiary hospitals in Hangzhou were investigated to provide reference for improving the practice level of evidence-based medicine.Methods:A total of 260 medical staff from tertiary hospitals in Hangzhou were investigated with the evidence-based practice scale.The structural equation modeling was used to analyze the direct and indirect effects of attitude,subjective norm,perceived behavior control and behavior intention on the practice behavior of evidence-based medicine,and to test the relationship among the influencing factors.Results:There were significant differences in evidence-based medicine practice behavior among medical staff with different educational backgrounds,professional titles,working years,practice intention and training participation(P<0.05).The structural equation modeling fitted well,and the behavioral intention had a direct positive impact on the practice behavior of evidence-based medicine(β=0.583,P<0.001),and attitude,subjective norm and perceived behavior control were indirectly affected by behavioral intention evidence-based medicine practice behavior of medical staff was affected(β=0.122,P<0.001;β=0.153,P<0.001;β=0.384,P<0.001).Conclusion:At present,the practice level of evidence-based medicine of medical staff needs to be improved.The practice behavior of medical staff should be improved from the aspects of improving the personal ability,cognition and practice level of medical staff,and strengthening social support for the practice of evidence-based medicine.
