摘  要:目的:以急性肺损伤/急性呼吸窘迫综合征(ALI/ARDS)新生儿为研究对象,分析患儿应用高频振荡通气(HFOV)与肺表面活性物质联合治疗及常频机械通气(CMV)与肺表面活性物质联合治疗的效果。方法:将2018年4月—2020年3月接受治疗的106例ALI/ARD新生儿分为对照组与观察组。予以对照组患儿CMV联合肺表面活性物质治疗,予以试验组患儿HFOV联合肺表面活性物质治疗,比较两组患儿治疗前后呼吸功能指标、用氧时间、机械通气时间以及并发症情况。结果:治疗后患儿各项呼吸力学指标均获得改善且试验组各项呼吸力学指标均优于对照组,差异有统计学意义(t=23.157,P<0.05)。试验组用氧时间及机械通气时间均短于对照组,差异有统计学意义(t=20.149,P<0.05)。试验组并发症发生率低于对照组,差异有统计学意义(χ^(2)=3.251,P<0.05)。结论:ALI/ARDS新生儿应用HFOV联合肺表面活性物质可取得确切疗效,有助于使患儿肺功能及呼吸功能获得显著改善,值得临床应用。Objective:The neonates with acute lung injury/acute respiratory distress syndrome(ALI/ARDS)were taken as the research object,to analyze the effects of combined treatment with high frequency oscillatory ventilation(HFOV)and pulmonary surfactant and combined treatment with constant frequency mechanical ventilation(CMV)and pulmonary surfactant.Methods:A total of 106 ALI/ARD neonates who received treatment from April 2018 to March 2020 were divided into control group and observation group.The children in the control group were treated with CMV combined with pulmonary surfactant,and the children in the experimental group were treated with HFOV combined with pulmonary surfactant.The respiratory function indexes,oxygen consumption time,mechanical ventilation time and complications were compared between the two groups before and after treatment.Results:After treatment,the respiratory mechanics indexes of the children were improved,and the respiratory mechanics indexes of the experimental group were better than those of the control group,the difference was statistically significant(t=23.157,P<0.05).The time of oxygen use and mechanical ventilation in the experimental group were shorter than those in the control group,and the difference was statistically significant(t=20.149,P<0.05).The incidence of complications in the experimental group was lower than that in the control group,and the difference was statistically significant(χ^(2)=3.251,P<0.05).Conclusion:The application of HFOV combined with pulmonary surfactant in ALI/ARDS neonates can achieve definite curative effect,which helps to significantly improve the pulmonary function and respiratory function of children,and is worthy of clinical application.
