中性粒细胞碱性磷酸酶检验的应用及诊断价值评估

摘　　要：目的评估在血液疾病诊断中检验中性粒细胞碱性磷酸酶的应用价值。方法方便选择于2019年6月—2020年5月就诊于该机构的血液疾病患者260例,并选择同期健康体检者74名,分为研究组和对照组,对血涂片进行制备,实施细胞化学染色,染色方法为偶氮偶联法(Kaplow),采用显微镜油镜观察中性细胞(成熟)100个,并记录观察结果。结果与对照组相比,研究组真性红细胞增多症、多发性骨髓瘤、慢性淋巴细胞性白血病、再生障碍性贫血、类白血病反应、急性淋巴细胞白血病患者的中性粒细胞碱性磷酸酶积分均明显较高,差异有统计学意义(t=27.442、36.067、10.276、8.792、73.531、19.736,P<0.05)。对照组中性粒细胞碱性磷酸酶积分(47.4±0.3)分,研究组睡眠性阵发性血红蛋白尿症积分(15.0±1.1)分、骨髓增生异常综合征(14.5±1.1)分、慢性髓细胞白血病患者(11.4±0.1)分、急性髓细胞白血病(19.3±0.5)分则明显较低,差异有统计学意义(t=172.286、233.970、699.894、390.811,P<0.05)。结论在血液各种疾病诊断中检验中性粒细胞碱性磷酸酶可获得满意的诊断效果,应用价值较高。Objective To evaluate the application value of neutrophil alkaline phosphatase in the diagnosis of blood diseases.Methods Conveniently selected from 260 patients with blood diseases who were treated in the institution from June 2019 to May 2020.And selected 74 healthy physical examination during the same period,and named them as the study group and the control group.The blood smear was prepared,and cytochemical staining was performed.The staining method was Kaplow.Observed 100 neutrophils(mature)with oil microscope and record the observation results.Results Compared with the control group,the neutrophil alkaline phosphatase scores of patients with polycythemia vera,multiple myeloma,chronic lymphocytic leukemia,aplastic anemia,leukemia reaction,and acute lymphoblastic leukemia in the study group increased significant higher,the difference was statistically significant(t=27.442,36.067,10.276,8.792,73.531,19.736,P<0.05).Compared with the control group neutrophil alkaline phosphatase scores(47.4±0.3)points,the study group sleep paroxysmal hemoglobinuria score(15.0±1.1)points,myelodysplastic syndrome(14.5±1.1)points,chronic myeloid leukemia patients(11.4±0.1)points,acute myeloid leukemia(19.3±0.5)points,being significantly lower,the difference was statistically significant(t=172.286,233.970,699.894,390.811,P<0.05).Conclusion The test of neutrophil alkaline phosphatase in the diagnosis of various blood diseases can obtain satisfactory diagnostic results and has high application value.
