快速康复外科理念(FTS)在老年股骨粗隆间骨折PFNA内固定术围术期临床护理研究

摘　　要：目的研究老年股骨粗隆间骨折患者在PFNA内固定术治疗围术期应用快速康复外科理念的临床效果。方法方便选取2020年1—12月期间于该院进行诊治的62例老年股骨粗隆间骨折患者,所有患者均接受PFNA内固定术治疗,利用抽签随机方式对患者予以分组,每组31例,其中参照组予以常规护理,研究组在快速康复外科理念下施以护理,观察并对比两组患者的护理效果。结果研究组护理满意度为93.5%,明显高于参照组,差异有统计学意义(χ^(2)=6.613,P=0.010);研究组并发症发生率为3.2%,明显低于参照组,差异有统计学意义(χ^(2)=4.679,P=0.030);研究组患者下床时间、住院时间分别为(4.13±1.24)、(11.23±2.36)d,优于参照组,差异有统计学意义(t=5.224、4.978,P<0.001);同时研究组患者护理后Barthel指数为(90.89±3.47)分,显著高于参照组的(81.25±3.33)分,差异有统计学意义(t=11.160,P<0.001)。结论老年股骨粗隆间骨折患者在PFNA固定术围术期应用快速康复外科理念能够改善患者临床症状,可以降低患者并发症发生率,提升其临床护理满意度,有助于促进患者身体康复和肢体功能的恢复,可在临床上大力推广。Objective To study the clinical effect of perioperative application of rapid recovery surgical concept in PFNA internal fixation for elderly patients with femoral intertrochanteric fractures.Methods 62 elderly patients with intertrochanteric fractures who were diagnosed and treated in the hospital from January to December 2020 were conveniently selected.All patients received PFNA internal fixation treatment.The patients were randomly divided into groups by lottery,with 31 cases in each group.Among them,the reference group was given routine nursing,and the research group was given nursing under the concept of rapid rehabilitation surgery.Observed and compared the nursing effects of the two groups of patients.Results The nursing satisfaction rate of the study group was 93.5%,which was significantly higher than that of the reference group,the difference was statistically significant(χ^(2)=6.613,P=0.010).The complication rate in the study group was 3.2%,which was significantly lower than that in the reference group,the difference was statistically significant(χ^(2)=4.679,P=0.030).The time to get out of bed and the length of stay in the study group were(4.13±1.24)d and(11.23±2.36)d,respectively,which was better than that in the reference group,the difference was statistically significant(t=5.224,4.978,P<0.001).At the same time,the Barthel index of the patients in the study group after nursing was(90.89±3.47)points,and was significantly higher than the(81.25±3.33)points of the reference group,the difference was statistically significan(t=11.160,P<0.001).Conclusion In elderly patients with femoral intertrochanteric fractures,applying the concept of rapid rehabilitation surgery during the perioperative period of PFNA fixation can improve the clinical symptoms of the patients.It can reduce the incidence of complications of patients and improve their clinical care satisfaction.It helps to promote the recovery of patients'body and limb function,and can be promoted clinically.
