七氟烷联合瑞芬太尼与单用七氟烷对麻醉后认知功能的影响对比

摘　　要：目的:比较分析瑞芬太尼联合七氟烷与单用七氟烷对患者麻醉后认知功能影响的差异,并探究瑞芬太尼联合七氟烷的临床效果。方法:选取2014年1月—2015年12月河南科技大学第一附属医院收治的200例疝修补术患者作为研究对象,并且依据对其麻醉用药的方案分为对照组(单用七氟烷)98例及实验组(七氟烷联合瑞芬太尼)102例。对患者的术前及拔管后的0.5 h、1 h、2 h与3 h的术后警觉评分(OAA/S)及简易智能状态测验(MMSE)进行评价。结果:相比于对照组,实验组拔管时间及苏醒时间明显较短,OAA/S评分及苏醒质量明显较高,差异有统计学意义(t=-34.12、11.076、-36.418,P<0.001);实验组在术后各时的MMSE评分均明显大于对照组,差异有统计学意义(t=-0.092、14.491、7.949、13.024、0.099,P<0.001),且在术后2 h时恢复至正常水平。结论:相比于单用七氟烷麻醉法,七氟烷联合瑞芬太尼麻醉法对患者术后认知功能的恢复及术后苏醒更有利。Objective:To explore the effects of sevoflurane combined with remifentanil on patients’cognitive function after anes⁃thesia.Methods:200 patients with hernia repair from January 2014 to December 2015 in the hospital were selected as the subjects.They were divided into control group(98 cases,sevoflurane)and experimental group(102 cases,remifentanil combined with sevoflu⁃rane).The postoperative wake-up quality(OAA/S score)and cognitive function(MMSE score)before operation and 0.5 h,1.0 h,2.0 h and 3.0 h after extubation were evaluated.Results:Compared with the control group,the extubation time and recovery time of the ex⁃perimental group were significantly shorter,and the OAA/S score and recovery quality were significantly higher,and the difference was statistically significant(t=-34.12,11.076,-36.418,P<0.001).The MMSE scores of the experimental group were significantly higher than those of the control group at all times after surgery(t=-0.092,14.491,7.949,13.024,0.009,P<0.001),and they returned to nor⁃mal levels at 2 h after surgery.Conclusion:Compared with sevoflurane,remifentanil combined with sevoflurane does better in recov⁃ery of cognitive function and revival after anesthesia.
