摘　　要：目的:探讨破裂前交通动脉动脉瘤(ACCA)患者经翼点入路夹闭术治疗预后的影响因素。方法:回顾性收集2015年3月—2019年3月偃师市中医院接受经翼点入路夹闭术治疗的142例破裂ACCA患者临床资料,采用格拉斯哥预后评分量表(GOS)对经翼点入路夹闭术治疗后的所有患者预后效果进行评估,根据评分结果,将分值为4~5分,设为预后良好组(n=97),1~3分,设为预后不良组(n=45)。仔细阅览142例ACCA患者临床资料,设计一般情况调查问卷,经单因素与多因素分析找出破裂ACCA患者经翼点入路夹闭术治疗预后的影响因素。结果:经多项非条件logistic回归分析检验结果显示,年龄≥60岁、术前Hunt-Hess分级≥Ⅲ级、合并高血压均是经翼点入路夹闭术治疗预后不良的影响因素(OR>1,P<0.05)。结论:破裂ACCA患者中,年龄≥60岁、术前Hunt-Hess分级≥Ⅲ级、合并高血压均是经翼点入路夹闭术治疗预后不良的影响因素,临床需重点关注。Objective:To investigate the prognostic factors of patients with ruptured anterior communicating aneurysm(ACCA)treated by pterional approach.Methods:The clinical data of 142 patients with ruptured ACCA treated by pterional approach in the hospital from March 2015 to March 2019 were retrospectively collected.The Glasgow Outcome Scale(GOS)was used to evalu⁃ate the prognosis of all patients after pterional approach clipping.According to the score results,patients with a score of 4-5 were set as a good prognosis group(n=97),and patients with a score of 1-3 were set as a poor prognosis group(n=45).After carefully reading the clinical data of 142 patients with ACCA,a general questionnaire was designed.By univariate and multivariate analysis,the influencing factors of prognosis of patients with ruptured ACCA treated by pterional approach were found out.Results:The re⁃sults of multiple unconditional logistic regression analysis showed that age≥60 years old,preoperative Hunt-Hess grade≥III,and hypertension were the influencing factors of poor prognosis of transpterional clipping(OR>1,P<0.05).Conclusion:In pa⁃tients with ruptured ACCA,age≥60 years old,preoperative Hunt-Hess grade≥III,and hypertension are the influencing factors of poor prognosis of transpterional clipping,which should be paid more attention in clinic.
