颈痛颗粒联合颈椎旁神经阻滞治疗颈源性头痛的临床研究

摘　　要：目的:探讨颈痛颗粒联合颈椎旁神经阻滞治疗颈源性头痛的临床效果。方法:选取2019年5月至2020年10月聊城市人民医院收治的颈源性头痛患者91例,根据随机数字表法分为对照组(颈椎旁神经阻滞治疗)45例和观察组(在对照组的基础上联合颈痛颗粒治疗)46例。观察两组患者的疗效、临床症状改善情况、颈部血流状况、炎症因子水平及不良反应发生情况。结果:观察组患者临床治疗的优良率为84.78%(39/46),明显高于对照组的62.22%(28/45),差异有统计学意义(P<0.05)。治疗后,观察组患者颈部平均血流速度较对照组更高;血管搏动指数、阻力指数,白细胞介素6、白细胞介素1β和肿瘤坏死因子α水平,疼痛数字评分、头痛发作频率较对照组更低,头痛持续时间较对照组更短,上述差异均有统计学意义(P<0.05)。结论:颈痛颗粒联合颈椎旁神经阻滞治疗颈源性头痛,可有效改善患者的临床症状,改善颈部血流状况,其作用机制可能与下调炎症因子有关。OBJECTIVE: To probe into the clinical efficacy of Jingtong granules combined with paracervical nerve block in the treatment of cervicogenic headache. METHODS: Totally 91 patients with cervicogenic headache admitted into Liaocheng People’s Hospital from May 2019 to Oct. 2020 were extracted to be divided into 45 cases in the control group(treated with paracervical nerve block) and 46 cases in the observation group(Jingtong granules combined with peripheral nerve block) via the random number table. The clinical efficacy, improvement of clinical symptoms, cervical blood flow, inflammatory factors and adverse drug reactions of two groups were observed. RESULTS: The excellent and good rate of clinical treatment in the observation group was 84.78%(39/46), which was significantly higher than 62.22%(28/45) in the control group, the difference was statistically significant(P<0.05).After treatment, the average blood flow velocity in the neck of the observation group was higher than that of the control group, and the vascular pulsation index, resistance index, interleukin 6, interleukin 1β, tumor necrosis factor α, pain digital score, and headache attack frequency were lower than those in the control group, the duration of headache was shorter than that of the control group, the difference was statistically significant(P<0.05). CONCLUSIONS: Jingtong granules combined with paracervical nerve block in the treatment of cervicogenic headache can effectively improve the clinical symptoms and cervical blood flow, and its mechanism may be related to the down-regulation of inflammatory cytokines.
