摘  要:目的探讨超声造影在小脉络膜肿瘤诊断中的临床应用价值。方法回顾性病例研究。经超声造影检查的42例(42只眼)小脉络膜肿瘤患者作为研究对象,以病理结果或眼科综合诊断且随访12个月为依据分为良性组25例,恶性组17例。应用SonoLiver软件分析超声造影动态资料,获得时间-强度(TI)曲线及相关定量数据。结果良性肿瘤TI曲线主要表现为快进慢出模式,恶性肿瘤TI曲线主要表现为快进快出模式。定量数据:峰值强度(IMAX)、上升时间(RT)、达峰时间(TTP)及平均渡越时间(mTT)恶性组低于良性组,差异具有统计学意义(P<0.05)。超声造影诊断小脉络膜肿瘤良恶性的符合率为88.1%(37/42)。结论超声造影检查可为小脉络膜肿瘤的鉴别诊断提供可靠的定量诊断依据。Objective To investigate the clinical application value of the contrast-enhanced ultrasound in the diagnosis of small choroidal tumors.Methods Retrospective case study.A total of 42 patients(42 eyes)with small choroidal tumors examined by contrast-enhanced ultrasound were retrospectively analyzed.The study participants were divided into the benign tumor group(25 eyes)and malignant tumor group(17 eyes)according to the pathological results or ophthalmic comprehensive diagnosis with 12 months follow-up.The SonoLiver software was used to analyze the dynamic data of contrast-enhanced ultrasound,the time intensity(TI)curve and the related quantitative data were obtained.Results The TI curve of benign tumors mainly showed a“fast in and slowly out”pattern,while that of malignant tumors mainly showed“fast in and fast out”.The maximum peak intensity(IMAX),rise time(RT),time to peak(TTP)and mean transit time(MTT)in the malignant group were lower than those in the benign group.The difference was statistically significant(P<0.05).The agreement of contrast-enhanced ultrasound in the diagnosis of small choroidal tumors was 88.1%(37/42).Conclusions Contrast-enhanced ultrasound can provide reliable quantitative diagnostic basis for the differential diagnosis of small choroidal tumors.
